重型β-地中海贫血患儿行为问题与家庭功能的相关性分析

摘　　要：目的了解重型β-地中海贫血患儿行为问题与家庭功能现状,并分析两者相关性,为构建健康服务管理模式及实施干预措施提供理论依据。方法于2022年9月—2023年4月采用一般资料调查表、Rutter儿童行为问卷(父母问卷)、家庭功能状况评估量表(FAD)对百色市某三级甲等医院141例重型β-地中海贫血患儿及其照顾者进行调查。结果重型β-地中海贫血患儿行为问题得分为16.44±6.47分;家庭功能总分为143.45±14.81分,处于“一般”水平;行为问题与家庭功能呈正相关(P<0.001);多重线性回归结果显示,性别、治疗方式、自理能力、家庭功能总分是影响重型β-地中海贫血患儿行为问题的影响因素(P<0.05)。结论重型β-地中海贫血患儿家庭功能和行为问题现状均有待改善,其中性别、治疗方式、自理能力、家庭功能总分是患儿行为问题的影响因素,这提示护理人员应关注家庭环境对患儿行为的影响,根据不同性别、自理能力对患儿提供个性化的干预措施,采取合适的治疗方式,以帮助改善其家庭功能,从而有效减少行为问题的发生,提高患儿的生活质量。Objective To understand the current status of behavioral problems and family functioning in children withβ-thalassemia major,and to analyze the correlation between the two,providing a theoretical basis for developing health service management models and implementing intervention measures.Methods From September 2022 to April 2023,a survey was conducted using a general information questionnaire,the Rutter Child Behavior Questionnaire(Parent Version),and the Family Assessment Device(FAD)on 141 children withβ-thalassemia major and their caregivers at a tertiary grade A hospital in Baise City.Results The behavioral problem score of children withβ-thalassemia major was 16.44±6.47 points;the total family functioning score was 143.45±14.81 points,which was at the"average"level,the behavioral problem was positively correlated with family function(P<0.001).Multiple linear regression analysis showed that the gender,treatment methods,self-care ability,and total family functioning scores are influencing factors for behavioral problems in children withβ-thalassemia major(P<0.05).Conclusion The status of family functioning and behavioral problems in children withβ-thalassemia major needs improvement.Gender,treatment methods,self-care ability,and total family functioning scores are influencing factors for behavioral problems in these children.This suggests that caregivers should pay attention to the impact of the family environment on the children's behavior,and provide personalized intervention measures for children based on different genders and self-care abilities,and adopt appropriate treatment approaches to help improve family functioning,effectively reducing the occurrence of behavioral problems and enhancing the quality of life for the children.
