桂枝茯苓汤合三黄汤联合达格列净治疗糖尿病周围神经病变的效果

摘　　要：目的探讨桂枝茯苓汤合三黄汤联合达格列净治疗糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)的效果。方法选取2022年1月—2024年4月江阴市中医外科医院收治的68例DPN患者为研究对象,按照不同用药方案分为两组,各34例。对照组采取常规疗法加达格列净治疗,观察组在对照组基础上加入桂枝茯苓汤合三黄汤治疗。比较两组治疗总有效率、血糖水平、胰岛素抵抗指数、胫神经和腓神经的感觉神经传导速度(sensory nerve conduction velocity,SNCV)、运动神经传导速度(motor nerve conduction velocity,MNCV)。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白、胰岛素抵抗指数水平均低于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组胫神经和腓神经的SNCV、MNCV均高于对照组,差异有统计学意义(P均<0.05)。结论桂枝茯苓汤合三黄汤联合达格列净治疗DPN的效果较好,能够有效控制患者的血糖水平,改善其神经传导功能。Objective To investigate the effect of Guizhi Fuling Decoction combined with Sanhuang Decoction and dapagliflozin in the treatment of diabetic peripheral neuropathy(DPN).Methods A total of 68 patients with DPN admitted to Jiangyin Traditional Chinese Medicine Surgery Hospital from January 2022 to April 2024 were selected as the research objects and divided into two groups according to different medication regimens,with 34 cases in each group.The control group was treated with conventional therapy plus dapagliflozin,and the observation group was treated with Guizhi Fuling Decoction and Sanhuang Decoction on the basis of the control group.The total effective rate of treatment,blood glucose levels,insulin resistance index,sensory nerve conduction velocity(SNCV)and motor nerve conduction velocity(MNCV)of tibial nerve and peroneal nerve were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c and insulin resistance index in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the SNCV and MNCV of tibial nerve and peroneal nerve in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion Guizhi Fuling Decoction combined with Sanhuang Decoction and dapagliflozin is effective in the treatment of DPN,which can effectively control blood glucose levels of patients and improve their nerve conduction function.
