妊娠期糖尿病患者血清SHBG、visfatin水平与胰岛素抵抗的关系

摘　　要：目的分析妊娠期糖尿病患者血清性激素结合球蛋白(sex hormone-binding globulin,SHBG)、内脏脂肪素(visfatin)水平与胰岛素抵抗的相关性。方法回顾性选取2022年3月—2023年3月在呼伦贝尔市妇幼保健院建档分娩的86例妊娠期糖尿病患者的临床资料为研究组,根据不同的胰岛素抵抗程度将研究组分为轻度组(38例)、中度组(26例)和重度组(22例);另选取同期本院的86例正常孕妇作为对照组。检测研究组与对照组、不同胰岛素抵抗严重程度患者的血清SHBG、visfatin水平,分析血清SHBG、visfatin与胰岛素抵抗水平的相关性。结果研究组血清SHBG水平低于对照组,visfatin水平高于对照组,差异有统计学意义(P均<0.05)。重度组血清SHBG水平低于中度组和轻度组,visfatin水平高于中度组和轻度组,差异有统计学意义(P均<0.05)。血清SHBG水平与妊娠期糖尿病患者胰岛素抵抗程度呈负相关,visfatin水平与胰岛素抵抗程度呈正相关。结论妊娠期糖尿病患者多伴随有胰岛素抵抗,且血清SHBG和visfatin水平与妊娠期糖尿病患者的胰岛素抵抗程度存在相关性。Objective To analyze the correlation between serum sex hormone-binding globulin(SHBG),visfatin levels and insulin resistance in patients with gestational diabetes mellitus.Methods The clinical data of 86 patients with gestational diabetes mellitus who were delivered in Hulunbeier Maternal and Child Health Hospital from March 2022 to March 2023 were retrospectively taken as the study group.According to different degrees of insulin resistance,the study group was divided into mild group(38 cases),moderate group(26 cases)and severe group(22 cases),and 86 normal pregnant women in our hospital during the same period were selected as the control group.The levels of serum SHBG and visfatin in the study group and the control group and patients with different severity of insulin resistance were detected,and the correlation between serum SHBG,visfatin and insulin resistance was analyzed.Results The level of serum SHBG in the study group was lower than that in the control group,and the level of visfatin was higher than that in the control group,and the differences were statistically significant(both P<0.05).The serum SHBG level of severe group was lower than that of moderate group and mild group,and the visfatin level was higher than that of moderate group and mild group,and the differences were statistically significant(both P<0.05).The level of serum SHBG was negatively correlated with the degree of insulin resistance in patients with gestational diabetes mellitus,and the level of visfatin was positively correlated with the degree of insulin resistance.Conclusion Patients with gestational diabetes mellitus are often accompanied by insulin resistance,and serum SHBG and visfatin levels are correlated with the degree of insulin resistance in patients with gestational diabetes mellitus.
