摘 要:目的研究2型糖尿病患者血清乳酸脱氢酶(lactate dehydrogenase,LDH)、空腹C肽(fasting C-peptide,FCP)、空腹血糖(fasting plasma glucose,FPG)、空腹胰岛素(fasting insulin,FINS)、胰岛β细胞功能指数(homeostatic model assessment ofβ-cell function,HOMA-β)水平与胰岛素抵抗的关系。方法选择2023年1月—2024年3月厦门大学附属龙岩中医院检验科收治的80例2型糖尿病患者为研究对象,根据患者每日需要补充的胰岛素总剂量(total daily insulin dose,TDID)分为3组,即A组[TDID为1.0~2.0 U(/kg·d),40例]、B组[TDID为>2.0~3.0 U(/kg·d),23例]、C组[TDID>3.0 U(/kg·d),17例]比较3组FPG、FINS、HOMA-β、LDH、FCP水平,并分析上述指标与胰岛素抵抗的相关性。结果C组FPG、LDH、FCP水平高于A组、B组,FINS、HOMA-β水平低于A组、B组,差异有统计学意义(P均<0.05)。2型糖尿病患者FPG、LDH、FCP与胰岛素抵抗呈正相关,FINS、HOMA-β与胰岛素抵抗呈负相关。结论2型糖尿病患者血清LDH、FCP、FPG、FINS、HOMA-β水平与胰岛素抵抗存在密切联系,对病情评价、治疗均有一定参考价值。Objective To study the relationship between serum lactate dehydrogenase(LDH),fasting C-peptide(FCP),fasting plasma glucose(FPG),fasting insulin(FINS),homeostatic model assessment ofβ-cell function(HOMA-β)and insulin resistance in type 2 diabetes mellitus patients.Methods A total of 80 patients with type 2 diabetes mellitus from January 2023 to March 2024 in the Department of Clinical Laboratory of Longyan Hospital of TCM Hospital Affiliated to Xiamen University were selected as the research objects.According to the total daily insulin dose(TDID),the patients were divided into group A[TDID was 1.0~2.0 U/(kg·d),40 cases],group B[TDID was>2.0~3.0 U/(kg·d),23 cases]and group C[TDID>3.0 U/(kg·d),17 cases].The levels of FPG,LDH,FCP,FINS and HOMA-βwere compared among the three groups,and the correlation between thses indexes levels and insulin resistance was analyzed.Results The levels of FPG,LDH and FCP in group C were higher than those in group A and group B,and the levels of FINS and HOMA-βwere lower than those in group A and group B,and the differences were statistically significant(all P<0.05).FPG,LDH and FCP were positively correlated with insulin resistance in patients with type 2 diabetes mellitus,while FINS and HOMA-βwere negatively correlated with insulin resistance.Conclusion Serum LDH,FCP,FPG,FINS,HOMA-βlevels are closely related to insulin resistance in patients with type 2 diabetes mellitus,which has certain reference significance for disease evaluation and treatment.
