早期胰岛素强化治疗对初诊2型糖尿病患者血糖控制效果的影响研究

摘　　要：目的探讨早期胰岛素强化治疗对初诊2型糖尿病患者的疗效。方法选择2020年1月—2022年1月福建省龙岩市博爱医院收治的200例2型糖尿病患者为研究对象,根据不同的治疗方法分为观察组与对照组,每组100例。观察组采用早期胰岛素强化治疗方式,对照组采用常规阶梯式治疗方式。比较两组血糖指标、胰岛功能指标、治疗效果。结果治疗后,两组空腹血糖和餐后2 h血糖均降低,且观察组低于对照组,差异有统计学意义(P均<0.05)。治疗后,两组胰岛素抵抗指数降低、胰岛β细胞功能指数升高,且观察组均优于对照组,差异有统计学意义(P均<0.05)。观察组治疗总有效率为98.00%(98/100),高于对照组的85.00%(85/100),差异有统计学意义(χ^(2)=10.865,P<0.05)。结论在治疗2型糖尿病过程中,实施早期胰岛素强化治疗具有明显的治疗效果,降低患者血糖指标,改善胰岛功能指标。Objective To investigate the therapeutic effect of early insulin on newly diagnosed patients with type 2 diabetes.Methods A total of 200 patients with type 2 diabetes admitted to Boai Hospital of Longyan City from January 2020 to January 2022 were selected as the research objects.According to different treatment methods,they were divided into observation group and control group,with 100 cases in each group.The observation group was treated with early intensive insulin therapy,and the control group was treated with conventional stepwise therapy.The blood glucose indexes,insulin function indexes and treatment effect were compared between the two groups.Results After treatment,the fasting plasma glucose and 2-hour postprandial plasma glucose of the two groups decreased,and the observation group was lower than the control group,the differences were statistically significant(all P<0.05).After treatment,the homeostatic model assessment of insulin resistance of the two groups decreased,and the homeostatic model assessment ofβ-cell function increased,and the observation group was better than the control group,the differences were statistically significant(all P<0.05).The total effective rate of the observation group was 98.00%(98/100),which was higher than 85.00%(85/100)of the control group,the difference was statistically significant(χ^(2)=10.865,P<0.05).Conclusion In the treatment of type 2 diabetes,the implementation of early intensive insulin therapy has obvious therapeutic effect,the patient's blood glucose index can be reduced and the insulin function index can be improved.
