摘　　要：目的探究糖尿病合并下肢静脉溃疡创面治疗中藻酸银敷料联合胰岛素的应用价值。方法回顾性选取2020年2月—2024年2月安溪县医院收治的80例糖尿病合并下肢静脉溃疡创面治疗的患者的临床资料,根据不同治疗方式分为常规组(藻酸银敷料联合二甲双胍治疗)、观察组(常规组基础上加入胰岛素治疗),各40例。对比两组血糖水平、胰岛功能及不良反应发生率。结果治疗90 d后,观察组空腹血糖、糖化血红蛋白、餐后2 h血糖水平均低于常规组,差异有统计学意义(P均<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。治疗90 d后,观察组胰岛β细胞功能指数高于常规组,胰岛素抵抗指数低于常规组,差异有统计学意义(P均<0.05)。结论糖尿病合并下肢静脉溃疡创面治疗中,在藻酸银敷料联合二甲双胍基础上加入胰岛素治疗可提高控糖效果,改善胰岛功能,且不会增加不良反应发生风险。Objective To explore the application value of silver alginate dressing combined with insulin in the treatment of diabetic lower extremity venous ulcer wounds.Methods The clinical data of 80 patients with diabetes mellitus complicated with lower extremity venous ulcer wound treated in Anxi County Hospital from February 2020 to February 2024 were retrospectively selected.According to different treatment methods,they were divided into routine group(treated with silver alginate dressing combined with metformin)and observation group(treated with insulin on the basis of routine group),with 40 cases in each group.The blood glucose levels,islet function and incidence of adverse reactions were compared between the two groups.Results After 90 days of treatment,the levels of fasting blood glucose,glycated hemoglobin A1c and 2-hour postprandial blood glucose in the observation group were lower than those in the routine group,and the differences were statistically significant(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).After 90 days of treatment,the isletβcell function index of the observation group was higher than that of the routine group,and the insulin resistance index was lower than that of the routine group,the differences were statistically significant(both P<0.05).Conclusion In the treatment of diabetic patients with lower extremity venous ulcer wounds,the addition of insulin on the basis of silver alginate dressing combined with metformin can improve the effect of glucose control,improve islet function and will not increase the risk of adverse reactions.
