多技术联合治疗糖尿病足溃疡的疗效分析

摘　　要：目的探讨分析封闭式负压吸引+水胶体硫酸银敷料+外用重组人酸性成纤维细胞生长因子(recombinant human acidic fibroblast growth factor,Rb-FGF)+植皮术治疗糖尿病足溃疡的临床疗效。方法回顾性选取2020年1月—2023年10月钦州市第一人民医院收治的96例糖尿病足溃疡患者的临床资料,按照不同治疗方案分为研究组(49例)与对照组(47例),研究组采用封闭式负压吸引+水胶体硫酸银敷料+Rb-FGF冲洗+植皮术,对照组采用封闭式负压吸引+水胶体硫酸银敷料+生理盐水冲洗+植皮术,分析比较两组患者住院时间、创面愈合时间、出院时愈合率及在治疗后的第3个月和第6个月的瘢痕评分。结果研究组住院时间、创面愈合时间均少于对照组,差异有统计学意义(P均<0.05)。研究组出院时愈合率高于对照组,差异有统计学意义(P<0.05)。治疗后第3个月和第6个月,研究组的瘢痕评分均低于对照组,差异有统计学意义(P均<0.05)。结论封闭式负压吸引+水胶体硫酸银敷料+Rb-FGF+植皮术治疗可促进糖尿病足溃疡愈合、缩短创面愈合时间及住院时间。Objective To investigate and analyze the clinical efficacy of closed negative pressure suction+hydrocolloid silver sulfate dressing+external application of recombinant human acidic fibroblast growth factor+skin grafting in the treatment of diabetes foot ulcer.Methods Clinical data of 96 patients with diabetic foot ulcer treated in the First People's Hospital of Qinzhou from January 2020 to October 2023 were retrospectively selected,and divided into study group(49 cases)and control group(47 cases)according to different treatment plans.The study group adopted closed negative pressure suction+hydrocolloid silver sulfate dressing+Rb-FGF irrigation+skin grafting.The control group was treated with closed negative pressure suction+hydrocolloid silver sulfate dressing+saline irrigation+skin grafting.The hospitalization time,wound healing time,healing rate at discharge and scar scores at the 3rd and 6th month after treatment were analyzed and compared between the two groups.Results The hospitalization time and wound healing time of the study group were lowe than those of the control group,and the differences were statistically significant(both P<0.05).The healing rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).At the 3rd and 6th month after treatment,the scar scores of the study group were lower than those of the control group,and the differences were statistically significant(both P<0.05).Conclusion Closed negative pressure suction+hydrocolloid silver sulfate dressing+Rb-FGF+skin grafting can significantly shorten the healing time of diabetes foot ulcer wounds and the hospitalization time.
