达格列净联合厄贝沙坦对早期糖尿病肾病患者糖代谢指标及肾功能的影响

摘　　要：目的探究达格列净联合厄贝沙坦在早期糖尿病肾病中的应用。方法选取2022年1月—2024年7月黔东南州人民医院收治的60例早期糖尿病肾病患者,按治疗方法不同分为达格列净组、厄贝沙坦组及联合用药组,各20例。比较3组糖代谢指标、肾功能指标及不良反应发生情况。结果治疗后,3组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均降低,且联合用药组均低于厄贝沙坦组、达格列净组,差异有统计学意义(P均<0.05)。治疗后,3组尿微量白蛋白肌酐比值、尿微量白蛋白水平均降低,且联合用药组均低于厄贝沙坦组、达格列净组,差异有统计学意义(P均<0.05)。3组不良反应发生率比较,差异无统计学意义(P>0.05)。结论达格列净联合厄贝沙坦治疗效果更佳,可更有效地调节早期糖尿病肾病患者糖代谢水平,改善肾功能,安全性较好。Objective To explore the application of dapagliflozin combined with irbesartan in early diabetic nephropathy.Methods Sixty patients with early diabetic nephropathy admitted to Qiandongnan Prefecture People's Hospital from January 2022 to July 2024 were selected and divided into dapagliflozin group,irbesartan group and combined medication group according to different treatment methods,with 20 cases each.Glucose metabolism indexes,renal function indexes and adverse reactions were compared among the three groups.Results After treatment,the levels of fasting plasma glucose,2-hour postprandial plasma glucose and glycated hemoglobin A1c in three groups were decreased,and the combined medication group were lower than irbesartan group and dapagliflozin group,and the differences were statistically significant(all P<0.05).After treatment,urinary microalbumin creatinine ratio and urinary microalbumin level were decreased in three groups,and combined medication group were lower than irbesartan group and dapagliflozin group,and the differences were statistically significant(both P<0.05).There was no significant difference in the incidence of adverse reactions among the three groups(P>0.05).Conclusion Dapagliflozin combined with irbesartan has more effective in regulating glucose metabolism levels and improving renal function in patients with early diabetes nephropathy,with better safety.
