王素霞教授应用通补奇经法治疗妊娠期阴道炎经验

摘　　要：阴道炎是指因阴道炎症导致女性外阴阴道出现症状如瘙痒、灼痛、异味、排尿困难和阴道分泌物变化的一组病症。本病是由感染或正常菌群失衡引起乳杆菌数量或比例减少,而细菌、霉菌、支原体、衣原体等病原微生物过度繁殖,进而导致阴道微生态紊乱造成的。阴道炎属于中医的带下病,是一种常见的妇科病,复发率高,给女性身心健康带来很大困扰。妊娠期阴道炎是指女性妊娠期间发生的阴道炎,可导致流产、早产、孕期绒毛膜炎、胎儿窘迫等严重不良后果。妊娠期可选择的抗菌药物非常有限,并且使用抗生素容易破坏正常的阴道菌群,故现代医学对妊娠期阴道炎尚无有效的根治方案。王素霞教授从医20余年,临床经验丰富,擅长以中医方法治疗各类妇科疾病。王素霞教授在总结前人经验及结合临床实际后,从中医奇经八脉理论出发,提出妊娠期阴道炎病属本虚标实,以正气不足、风湿内侵,致使奇经损伤为主要病机。王素霞教授提出采用中药外治法结合内服法治疗妊娠阴道炎,标本兼治,有除湿止带并调补奇经、安胎之功,临床疗效显著。本研究总结王素霞教授治疗妊娠期阴道炎学术思想及临证经验,并附带验案一则,以飨同道。Vaginitis refers to inflammation of the vagina,which is a group of diseases that cause vaginal infection symptoms such as itching,burning,unusual odor,difficulty urinating,and changes in vaginal discharge in women.This disease is caused by infection or imbalance of the normal flora due to the reduction in the number or proportion of normal lactobacilli,and bacteria,mold,mycoplasmas,chlamydia,and others pathogenic microorganisms overreproduction,resulting in vaginal microecological disorders.In traditional Chinese medicine(TCM),vaginitis is categorized under Daixia disease(带下病),a common gynecological disease with a high recurrence rate,significantly affecting women’s physical and mental health.Gestational vaginitis refers to vaginitis occurring during pregnancy,which can lead to miscarriage,preterm birth,pregnancy chorionic inflammation,fetal distress,and other serious adverse outcomes.Due to the limited choice of antibacterial drugs during pregnancy and the tendency of antibiotics to disrupt the normal vaginal flora,there is currently no effective radical treatment for gestational vaginitis.Professor WANG Suxia,with over 20 years of medical practice,excels in treating various gynecological diseases by TCM therapies.After summarizing the experiences of predecessors and combining with clinical practice,and starting from the theory of eight veins of extraordinary meridians in TCM,Professor WANG Suxia proposes that gestational vaginitis falls under the concept of deficiency in origin and excess in superficiality in TCM,and the main pathogenesis is invasion of winddampness after insufficiency of Zhengqi(vital energy),leading to the damage of extraordinary meridians.Professor WANG Suxia proposes that Chinese medicines external application plus internal administration can be used to treat gestational vaginitis,so as to address both symptoms and root causes.This therapy has the functions of removing damp and arresting morbid leucorrhea,regulating and replenishing extraordinary meridians,and preventing miscarriage
