补肾活血中药在先兆流产合并宫腔积液中的应用进展

摘　　要：先兆流产是一种常见的妊娠并发症,发病率较高。临床研究发现,先兆流产与宫腔积液存在一定的相关性。宫腔积液增多、范围持续增大可刺激子宫收缩,影响胚胎发育,导致流产。现代医学认为,宫腔积液的成分及形成原因尚不明确,绒毛膜下血肿形成、激素水平不稳定、免疫功能异常、凝血功能失常、炎症反应等均可导致宫腔积液发生。研究从先兆流产合并宫腔积液的病因病机出发,论述宫腔积液与血瘀的关系,探讨补肾活血类中药在治疗先兆流产合并宫腔积液中的应用情况。本病病机为“肾虚血瘀”,采用补肾活血药物治疗。对于妊娠期女性,在应用补肾活血药物时应遵循“有故无殒,亦无殒也”的原则。研究分别从理论依据、作用机制、临床应用现状等方面进行阐述,证实其有效性及安全性。补肾活血类药物在治疗先兆流产合并宫腔积液方面具有提高妊娠激素水平、改善血液循环、调节免疫机制等作用,可促进宫腔积液吸收,改善患者的临床症状,提高保胎成功率,临床取得较好疗效,值得深入研究及推广应用。Threatened abortion is a common complication of pregnancy with a high incidence.Clinical studies have found that there is a certain correlation between threatened abortion and uterine effusion.The increase of uterine effusion and the continuous enlargement of its scope can stimulate uterine contraction,affect embryonic development,and lead to abortion.In modern medicine,the composition and cause of uterine effusion are still unclear.The formation of subchorionic hematoma,unstable hormone level,Clinical Journal of Chinese Medicine 2024-60-Vol.(16)No.29 abnormal immune function,abnormal coagulation function,and inflammatory reaction can all lead to the occurrence of uterine effusion.Based on the etiology and pathogenesis of threatened abortion plus intrauterine effusion,the relationship between intrauterine effusion and blood stasis is discussed in this paper to summarize the application of Chinese medicines for tonifying kidney and promoting blood circulation in treating threatened abortion plus uterine effusion.The pathogenesis of the disease is kidney deficiency and blood stasis.Based on the pathogenesis,Chinese medicines for tonifying kidney and promoting blood circulation are used for treatment.For pregnant women,the principle of‘treating from the aspect of pathogenesis does not lead to abortion’should be followed in the application of kidneytonifying and blood-activating drugs.In this paper,the theoretical basis,action mechanism,and clinical application status are discussed respectively to confirm its effectiveness and safety.In the treatment of threatened abortion plus uterine effusion,Chinese medicines for tonifying kidney and promoting blood circulation has the effects of improving pregnancy hormone level,improving blood circulation,and regulating immune mechanism,can promote the absorption of uterine effusion,relieve the clinical symptoms of patients,and improve the success rate of fetal protection,with good clinical efficacy,which is worthy of further study and promotion.
