摘 要:目的评价肺结核合并糖尿病(diabetes mellitus,DM)患者接受强化血糖控制治疗的效果及有效率。方法选取2023年1月—2024年6月中国十九冶集团有限公司职工医院收治的60例肺结核合并DM患者为研究对象,按治疗方法不同分为对照组(常规治疗)与观察组(强化血糖控制治疗),每组30例。比较两组治疗总有效率、血糖水平、胰岛功能、血清炎性因子、不良反应发生情况。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组糖化血红蛋白、空腹血糖、餐后2 h血糖水平均低于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组胰岛β细胞功能指数、胰岛素抵抗指数、红细胞沉降率、超敏C反应蛋白水平均优于对照组,差异有统计学意义(P均<0.05)。观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论强化血糖控制治疗可增强肺结核合并DM患者的抗结核效果及血糖控制效果,改善其胰岛功能,减轻炎症反应,减少不良反应发生率。Objective To evaluate the effect and efficiency of intensive blood glucose control in patients with pulmonary tuberculosis complicated with diabetes mellitus(DM).Methods Sixty patients with tuberculosis complicated with DM admitted to the Staff Hospital of China 19th Metallurgical Group Co.,Ltd.from January 2023 to June 2024 were selected as the study objects.According to different treatment methods,they were divided into control group(conventional treatment)and observation group(intensive blood glucose control treatment),with 30 cases in each group.The total effective rate of treatment,blood glucose level,islet function,serum inflammatory factors and adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of glycated hemoglobin A1c,fasting plasma glucose and 2-hour postprandial plasma glucose in observation group were lower than those in control group,and the differences were statistically significant(all P<0.05).After treatment,the levels of homeostasis model assessment-β,homeostasis model assessment-insulin resistance,erythrocyte sedimentation rate and hypersensitive C-reactive protein in observation group were better than those in control group,and the differences were statistically significant(all P<0.05).The incidence of adverse reactions in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion Intensive blood glucose control therapy can enhance the anti-tuberculosis effect and blood glucose control effect of DM patients with pulmonary tuberculosis,improve their islet function,reduce inflammation and reduce the incidence of adverse reactions.
