摘 要:目的探究妊娠期糖尿病患者采用胰岛素泵和多次皮下注射胰岛素治疗的临床疗效及对血糖代谢的影响。方法选取2021年1月—2023年12月泉州市第一医院N19区妇科收治的92例妊娠期糖尿病患者,根据不同的治疗方法分为对照组(n=46,多次皮下注射胰岛素治疗)、观察组(n=46,胰岛素泵治疗)。对比两组血糖水平、血糖达标时间、胰岛素用量、胰高血糖素与胰岛素水平。结果治疗前,两组血糖水平比较,差异无统计学意义(P均>0.05);治疗后,两组血糖水平均比治疗前降低,且观察组空腹血糖、餐后2 h血糖均低于对照组,差异有统计学意义(P均<0.05)。观察组血糖达标时间短于对照组,胰岛素用量少于对照组,差异有统计学意义(P均<0.05)。治疗前,两组胰高血糖素、胰岛素水平比较,差异无统计学意义(P均>0.05);治疗后,观察组胰高血糖素、胰岛素水平低于对照组,差异有统计学意义(P均<0.05)。结论在妊娠期糖尿病治疗中,相比多次皮下注射胰岛素治疗,胰岛素泵治疗效果更佳,可控制降低血糖,缩短血糖达标时间,减少胰岛素用量,改善血糖代谢。Objective To explore the clinical efficacy of insulin pump and multiple subcutaneous injections of insulin in the treatment of patients with gestational diabetes mellitus and the impact on blood glucose metabolism.Methods From January 2021 to December 2023,a total of 92 patients with gestational diabetes mellitus admitted to the Department of Gynecology,N19 District in Quanzhou First Hospital were selected,according to different treatment methods,they were divided into control group(n=46,multiple subcutaneous insulin injection treatment)and observation group(n=46,insulin pump treatment).The blood glucose levels,blood glucose compliance time,insulin dosage,glucagon and insulin levels were compared between the two groups.Results Before treatment,there were no significant differences in blood glucose levels between the two groups(both P>0.05).After treatment,the blood glucose levels of the two groups were lower than those before treatment,and the fasting plasma glucose and 2-hour postprandial plasma glucose of the observation group were lower than those of the control group,the differences were statistically significant(both P<0.05).The blood glucose compliance time in the observation group was shorter than that in the control group,and the dosage of insulin was less than that in the control group,the differences were statistically significant(both P<0.05).Before treatment,there were no significant differences in glucagon and insulin levels between the two groups(both P>0.05).After treatment,the glucagon and insulin levels in the observation group were lower than those in the control group,and the differences were statistically significant(both P<0.05).Conclusion In the treatment of gestational diabetes mellitus,compared with multiple subcutaneous injections of insulin,the effect of insulin pump treatment is better,which can control and reduce blood glucose,shorten the blood glucose compliance time,reduce the amount of insulin,and improve blood glucose metabolism.
