沙格列汀联合盐酸二甲双胍片对老年2型糖尿病患者血糖、血脂代谢及胰岛功能的影响

摘　　要：目的探究沙格列汀联合盐酸二甲双胍片对老年2型糖尿病患者血糖、血脂代谢及胰岛功能的影响。方法回顾性选取2022年3月—2024年3月江苏省人民医院宿迁医院收治的89例老年2型糖尿病患者的临床资料,根据不同的治疗方法分为对照组(43例,接受盐酸二甲双胍片治疗)和观察组(46例,联合使用沙格列汀与盐酸二甲双胍片治疗)。对比治疗前后两组血糖、血脂及胰岛功能指标。结果治疗前,两组患者血糖、血脂及胰岛功能指标比较,差异无统计学意义(P均>0.05)。治疗后,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白、甘油三酯、总胆固醇、低密度脂蛋白胆固醇水平均低于对照组,高密度脂蛋白胆固醇水平高于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组胰岛素抵抗指数低于对照组,胰岛β细胞功能指数与空腹胰岛素水平均高于对照组,差异有统计学意义(P均<0.05)。结论沙格列汀联合盐酸二甲双胍片治疗老年2型糖尿病患者能更有效地改善血糖、血脂及胰岛功能,改善胰岛素抵抗,提升治疗效果。Objective To investigate the effects of saxagliptin combined with metformin hydrochloride tablets on blood glucose,lipid metabolism,and islet function in elderly patients with type 2 diabetes mellitus.Methods The clinical data of 89 elderly patients with type 2 diabetes mellitus admitted to Jiangsu Province(Suqian)Hospital from March 2022 to March 2024 were retrospectively selected.According to different treatment methods,they were divided into control group(43 cases,treated with metformin hydrochloride tablets)and observation group(46 cases,treated with saxagliptin combined with metformin hydrochloride tablets).The blood glucose,blood lipid and islet function indexes of the two groups before and after treatment were compared.Results Before treatment,there was no significant difference in blood glucose,blood lipid and islet function between the two groups(all P>0.05).After treatment,the levels of fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c,triglyceride,total cholesterol and low density lipoprotein cholesterol in the observation group were lower than those in the control group,and the level of high density lipoprotein cholesterol was higher than that in the control group,the differences were statistically significant(all P<0.05).After treatment,the insulin resistance index of the observation group was lower than that of the control group,and the islet β cell function index and fasting insulin level were higher than those of the control group,the differences were statistically significant(all P<0.05).Conclusion Saxagliptin combined with metformin hydrochloride tablets in the treatment of elderly patients with type 2 diabetes mellitus can more effectively improve blood glucose,blood lipids and islet function,significantly improve insulin resistance,and improve the therapeutic effect.
