摘 要:目的:观察420 nm强脉冲光联合长脉冲1064 nm Nd∶YAG激光治疗玫瑰痤疮的疗效。方法:选取2020年6月-2023年7月笔者医院收治的玫瑰痤疮患者150例。随机分为A组、B组及C组,各50例,A组行420 nm强脉冲光治疗,B组用长脉冲1064 nm Nd∶YAG激光治疗,C组为420 nm强脉冲光联合长脉冲1064 nm Nd∶YAG激光治疗。比较三组患者临床疗效、皮损症状积分、皮肤屏障功能、不良反应、皮肤病生活质量指数(DLQI)量表。结果:C组临床有效率比较96.00%vs.80.00%vs.82.67%,C组高于A组及B组(P<0.05);治疗后,三组皮损症状评分均有所下降,且C组各项积分低于A组及B组(P<0.05);治疗后,三组红斑面积、经皮水分流失量(TEWL)水平均有所下降,且C组低于A组及B组,皮肤角质层含水量均有所升高,且C组高于A组及B组(P<0.05);C不良反应发生率8.00%与B组6.00%及A组10.00%比较,差异无统计学意义(P>0.05);治疗8周后,三组DLQI评分均有所降低,且C组低于A组及B组(P<0.05)。结论:420 nm强脉冲光联合长脉冲1064 nm Nd∶YAG激光治疗玫瑰痤疮疗效确切,有助于改善皮损及皮肤屏障功能,提高患者生活质量,且具有安全性。Objective To observe the cosmetic efficacy of 420 nm intense pulsed light combined with long-pulsed 1,064 nm Nd∶YAG laser in the treatment of rosacea.Methods A total of 150 patients with rosacea admitted to the hospital between June 2020 and July 2023 were enrolled and randomly divided into three groups:group A(420 nm intense pulsed light),group B(long-pulsed 1,064 nm Nd∶YAG laser),and group C(420 nm intense pulsed light combined with long-pulsed 1,064 nm Nd∶YAG laser),with 50 patients in each group.Clinical efficacy,scores of skin lesion symptoms,skin barrier function,adverse reactions,and dermatology life quality index(DLQI)scores were compared among the three groups..Results The clinical response rate in group C was higher than that in group A and group B(96.00%vs.80.00%vs.82.67%,P<0.05).After treatment,the scores for skin lesion symptoms decreased in all three groups,with group C showing lower scores compared to group A and group B(P<0.05).The number of erythematous lesions and transepidermal water loss(TEWL)decreased in all groups,with group C showing more significant reductions than groups A and B,while the water content of the stratum corneum increased,being higher in group C than in group A and group B(P<0.05).The incidence of adverse reactions did not differ significantly among the three groups(8.00%vs.6.00%vs.10.00%,P>0.0).After 8 weeks of treatment,DLQI scores decreased in all three groups,with group C showing lower scores than group A and group B(P<0.05).Conclusion The combination of 420 nm intense pulsed light and long-pulsed 1,064 nm Nd∶YAG laser demonstrates significant clinical efficacy in rosacea patients.This combination therapy effectively improves skin lesion symptoms,enhances skin barrier function,and improves the quality of life,with a favorable safety profile.
