胰岛素在急诊治疗糖尿病酮症酸中毒患者中的应用分析

摘　　要：目的探究在急诊糖尿病酮症酸中毒治疗中应用胰岛素治疗的效果。方法回顾性选取2023年1月—2024年6月中国人民解放军陆军第七十三集团军医院收治的130例糖尿病酮症酸中毒患者的临床资料,按照治疗方案不同分为两组,各65例。对照组实施小剂量胰岛素静脉滴注治疗,观察组实施持续胰岛素泵入治疗。对比两组血糖水平、临床康复指标及并发症发生率。结果观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。观察组中毒症状缓解时间及尿酮体转阴时间均短于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组空腹血糖及餐后2 h血糖水平均低于对照组,差异有统计学意义(P均<0.05)。结论实施胰岛素治疗可改善急诊糖尿病酮症酸中毒患者的血糖水平,缓解酸中毒症状,提升整体治疗效率。Objective To explore the effect of insulin therapy in the treatment of emergency diabetic ketoacidosis.Methods The clinical data of 130 patients with diabetic ketoacidosis admitted to the 73rd Army Group Hospital of the PLA from January 2023 to June 2024 were retrospectively selected and divided into two groups according to different treatment regimens,with 65 cases in each group.The control group was treated with low-dose insulin intravenous drip,and the observation group was treated with continuous insulin pump.The blood glucose level,clinical rehabilitation index and complication rate were compared between the two groups.Results The incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).The remission time of poisoning symptoms and the time of urine ketone body turning negative in the observation group were shorter than those in the control group,and the differences were statistically significant(both P<0.05).After treatment,the levels of fasting blood glucose and 2-hour postprandial blood glucose in the observation group were lower than those in the control group,and the differences were statistically significant(both P<0.05).Conclusion The implementation of insulin therapy can significantly improve the blood glucose level of patients with emergency diabetic ketoacidosis,relieve the symptoms of acidosis,and improve the overall treatment efficiency.
