摘  要:目的 研究对早产儿施以氨基酸初始大剂量静脉营养的效果,及对其营养状态及身体发育情况的影响。方法 非随机选取2022年8月—2024年5月东莞市长安医院儿科收治的160例早产儿为研究对象。根据营养方式的不同,将其分为对照组(氨基酸初始低剂量静脉营养治疗)和研究组(氨基酸初始大剂量静脉营养治疗),各80例。比较两组早产儿的身体发育状况、营养状况、代谢指标、治疗1周内氮平衡及并发症。结果研究组早产儿的头围增长、体质量增加情况优于对照组,最大体质量下降、恢复至正常出生体质量时间、静脉营养使用时间均低于对照组,差异有统计学意义(P均<0.05)。治疗后,研究组的营养状况指标均优于对照组,差异有统计学意义(P均<0.05)。研究组24 h尿素氮水平高于对照组,差异有统计学意义(P<0.05)。研究组1周内氮平衡均高于对照组,差异有统计学意义(P均<0.05)。研究组早产儿并发症总发生率为3.75%(3/80),低于对照组的13.75%(11/80),差异有统计学意义(χ^(2)=5.010,P=0.025)。结论 对早产儿实施氨基酸初始大剂量静脉营养支持的效果更好,对其营养状态改善及身体发育均有促进作用,且对肾脏功能影响较小,可改善负氮平衡,并发症少,安全性高。Objective To study the effect of initial high-dose of intravenous nutrition in premature infants and the effect on their nutritional status and physical development.Methods A total of 160 premature infants admitted to the Department of Pediatrics,Chang′an Hospital of Dongguan City from August 2022 to May 2024 were non-randomly selected as the research objects.According to the different nutritional methods,they were divided into control group(amino acid initial low-dose intravenous nutrition therapy)and study group(amino acid initial high-dose intravenous nutrition therapy),80 cases in each group.The physical development,nutritional status,metabolic indexes,nitrogen balance and complications within 1 week of treatment were compared between the two groups.Results The head circumference growth and weight gain of premature infants in the study group were better than those in the control group,the time of maximum weight loss,recovery to normal birth weight and intravenous nutrition use were lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the nutritional status indexes of the study group were better than those of the control group,and the differences were statistically significant(all P<0.05).The 24 h urea nitrogen level in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).The nitrogen balance in the study group were higher than those in the control group within 1 week,and the differences were statistically significant(all P<0.05).The total incidence of complications in the study group was 3.75% (3/80), which was lower than 13.75% (11/80) in the control group, and the difference was statistically significant (χ2=5.010, P=0.025).
