胸腺肽α1联合纤支镜肺灌洗治疗重症肺炎合并脓毒症的临床效果及对细胞因子的影响

摘　　要：目的 探讨胸腺肽α1联合纤维支气管镜灌洗治疗重症肺炎合并脓毒症的疗效。方法 非随机选取2020年1月—2024年2月徐州医科大学附属医院收治的80例重症肺炎合并脓毒症患者为研究对象,按治疗方式不同分两组,各40例。两组患者均采用抗感染、吸氧等常规治疗,对照组采用纤支镜肺泡灌洗,观察组在此基础上皮下注射胸腺肽α1,比较两组疗效、生命体征、血气分析、细胞因子。结果 观察组临床有效率为92.50%(37/40),高于对照组的72.50%(29/40),差异有统计学意义(χ^(2)=4.242,P<0.05)。观察组的呼吸频率、心率、动脉二氧化碳分压、白细胞介素6、肿瘤坏死因子α均低于对照组,经皮氧饱和度、酸碱度、动脉血氧分压/氧浓度、白细胞介素10均高于对照组,差异有统计学意义(P均<0.05)。结论 胸腺肽α1联合纤维支气管镜肺灌洗可以改善重症肺炎合并脓毒症患者临床症状,抑制炎症进展,提高临床疗效。Objective To investigate the curative effect of thymosinα1 combined with bronchoscopic lavage in the treatment of severe pneumonia complicated with sepsis.Methods A total of eighty patients with severe pneumonia complicated with sepsis admitted to the Affiliated Hospital of Xuzhou Medical University from January 2020 to February 2024 were non-randomly selected as the study objects and divided into two groups according to different treatment methods,with forty cases in each group.Both groups were treated with anti-infection,oxygen inhalation and other conventional treatments;the control group was treated with bronchoscopic alveolar lavage,and the observation group was injected with thymosin α1 under the basal epithelia.The efficacy,vital signs,blood gas analysis and cytokines of the two groups were compared.Results The clinical effective rate of observation group was 92.50%(37/40),which was higher than 72.50%(29/40)of control group,and the difference was statistically significant(χ2=4.242,P<0.05).Respiratory rate,heart rate,arterial carbon dioxide pressure,interleukin 6 and tumor necrosis factorαin observation group were lower than those in control group,while transcutaneous oxygen saturation,pH,partial arterial oxygen pressure/fraction of inspire oxygen and interleukin 10 were higher than those in control group,with statistical significance(all P<0.05).Conclusion Thymosinα1 combined with bronchoscopic lung lavage can improve the clinical symptoms of severe pneumonia complicated with sepsis,inhibit the progression of inflammation and improve the clinical efficacy.
