声带息肉摘除术中应用瑞芬太尼复合丙泊酚麻醉对患者血流动力学指标的影响

摘　　要：目的 分析瑞芬太尼+丙泊酚在声带息肉摘除术中的麻醉效果。方法 非随机选取2022年1月—2024年1月浙江省人民医院毕节医院收治的84例声带息肉患者作为研究对象,按照麻醉方式不同分为两组,各42例。对照组给予芬太尼+丙泊酚麻醉,观察组给予瑞芬太尼+丙泊酚麻醉。对比两组患者血流动力学指标、麻醉相关指标与不良反应。结果 T1时刻,对照组的收缩压、舒张压、心率均低于观察组,差异有统计学意义(P均<0.05)。T2时刻,对照组的收缩压、舒张压均低于观察组,差异有统计学意义(P均<0.05)。观察组丙泊酚的用量低于对照组、时间性指标均短于对照组,差异有统计学意义(P均<0.05)。观察组不良反应发生率为4.76%(2/42),低于对照组的21.43%(9/42),差异有统计学意义(χ^(2)=5.126,P<0.05)。结论 在声带息肉摘除手术当中采用瑞芬太尼联合丙泊酚麻醉,可使手术中血流动力学稳定,减少麻醉药用量,减少不良反应。Objective To analyze the anesthetic effect of remifentanil+propofol in vocal cord polypectomy.Methods From January 2022 to January 2024,eighty-four patients with voval cord polyps admitted to Bijie Hospital of Zhejiang Provincial People′s Hospital were non-randomly selected as the research objects.They were divided into two groups according to different anesthesia methods,with forty-two cases in each group.The control group was given fentanyl+propofol anesthesia,and the observation group was given remifentanil+propofol anesthesia.The hemodynamic indexes,anesthesia related indexes and adverse reactions were compared between the two groups.Results At T1,the systolic blood pressure,diastolic blood pressure and heart rate of the control group were lower than those of the observation group,and the differences were statistically significant(all P<0.05).At T2,the systolic blood pressure and diastolic blood pressure in the control group were lower than those in the observation group,and the differences were statistically significant(both P<0.05).The dosage of propofol in the observation group was lower than that in the control group,and the time index was shorter than that in the control group,the differences were statistically significant(all P<0.05).The incidence of adverse reactions in the observation group was 4.76%(2/42),which was lower than 21.43%(9/42)in the control group,and the difference was statistically significant(χ2=5.126,P<0.05).Conclusion Remifentanil combined with propofol anesthesia in vocal cord polypectomy can stabilize hemodynamics during surgery,reduce the dosage of anesthetics and reduce adverse reactions.
