摘　　要：目的 分析5分钟紧急剖宫产术后产妇及围产儿不良结局的影响因素。方法 非随机选取2021年10月—2023年12月在东莞市厚街医院行5分钟紧急剖宫产的30例产妇为研究对象,按妊娠结局不同将研究对象分为两组,正常结局组(23例)与不良结局组(7例)。通过单因素和多因素Logistic回归分析产妇及围产儿结局的影响因素。结果 单因素分析结果显示,不良结局组瘢痕子宫、妊娠期高血压、妊娠期糖尿病、前置胎盘、巨大儿占比高于正常结局组,差异有统计学意义(P均<0.05)。多因素Logistic分析结果显示,瘢痕子宫、妊娠期高血压、妊娠期糖尿病、前置胎盘、巨大儿是5分钟紧急剖宫产术后产妇及围产儿不良结局的独立危险因素(OR=4.792、4.968、4.792、4.792、4.792,P均<0.05)。结论 瘢痕子宫、妊娠期高血压、妊娠期糖尿病、前置胎盘、巨大儿是5分钟紧急剖宫产不良妊娠结局的危险因素,参考这些高危因素对高危产妇采取预防、预警、应急等措施以降低不良妊娠结局的发生。Objective To analyze the influencing factors of maternal and perinatal adverse outcomes after 5-minute emergency cesarean section.Methods Non-randomly selected 30 puerpera who underwent 5-minute emergency cesarean section in Houjie Hospital of Dongguan City from October 2021 to December 2023 as the study subjects.The subjects were divided into two groups according to different pregnancy outcomes,normal outcome group(twenty-three cases)and adverse outcome group(seven cases).Univariate and multivariate Logistic regression analysis were used to analyze the influencing factors of maternal and perinatal outcomes.Results The of univariate analysis showed that the ratio of scar uterus,gestational hypertension,gestational diabetes,placenta previa and macrosomia in the adverse outcome group were higher than those in the normal outcome group,and the differences were statistically significant(all P<0.05).Multivariate Logistic analysis showed that scar uterus,gestational hypertension,gestational diabetes,placenta previa,and macrosomia were independent risk factors for maternal and perinatal adverse outcomes after 5-minute emergency cesarean section(OR=4.792,4.968,4.792,4.792,4.792,all P<0.05).Conclusion Scar uterus,gestational hypertension,gestational diabetes,placenta previa,and macrosomia are risk factors for adverse pregnancy outcomes of 5-minute emergency cesarean section.These high-risk factors can be used to take prevention,early warning,and emergency measures for high-risk women to reduce the occurrence of adverse pregnancy outcomes.
