髌上入路多向锁定带锁髓内钉固定术在胫骨近端骨折患者中的应用效果

摘　　要：目的:探究髌上入路多向锁定带锁髓内钉固定术在胫骨近端骨折患者中的应用效果。方法:回顾性分析2017年1月—2023年10月永宁县人民医院收治的50例胫骨近端骨折患者资料,根据手术方式的不同将其分为对照组和研究组,各25例。对照组采用锁定接骨板固定术治疗,研究组采用髌上入路多向锁定带锁髓内钉固定术治疗。对比两组围手术期指标、关节功能恢复状况及并发症发生情况。结果:研究组手术持续时间、术后主动直腿抬高时间、骨折愈合时间均短于对照组,透视次数、术中出血量均少于对照组,差异均有统计学意义(P<0.05)。术后6个月,两组膝关节Lysholm评分及美国足踝外科协会踝与后足评分均高于术前,且研究组均高于对照组,差异均有统计学意义(P<0.05)。术后6个月,研究组并发症发生率为4.00%,低于对照组的28.00%,差异有统计学意义(P<0.05)。结论:髌上入路多向锁定髓内钉固定术在治疗胫骨近端骨折患者中,可有效改善其围手术期指标,促进术后关节功能恢复,并降低术后并发症的发生率。Objective:To investigate the effect of suprapatellar approach multidirectional locking intramedullary nail fixation in patients with proximal tibial fractures.Methods:Data of 50 patients with proximal tibial fracture admitted to Yongning County People’s Hospital from January 2017 to October 2023 were retrospectively analyzed as the study objects.They were divided into a control group and a study group according to different surgical methods,with 25 cases each group.The control group was treated with locking bone plate fixation,the study group was treated with suprapatellar approach multidirectional locking intramedullary nail fixation.Perioperative indexes,joint function recovery and the occurrence of complications were compared between the two groups.Results:The operation duration,active straight leg elevation time and fracture healing time of the study group were all shorter than those of the control group,the perspective frequency and intraoperative blood loss were all less than those of the control group,with statistical significance(P<0.05).Six months after surgery,the Knee Lysholm Score and American Orthopedic Foot and Ankle Society Ankle-Hindfoot Scale score in both groups were higher than those before surgery,the study group were higher than the control group,with statistical significance(P<0.05).Six months after operation,the complication rate of the study group was 4.00%,which was lower than 28.00%of the control group,the difference was statistically significant(P<0.05).Conclusion:Suprapatellar approach multidirectional locking intramedullary nail fixation with suprapatellar approach can effectively improve perioperative indexes in patients with proximal tibial fractures,promote postoperative joint function recovery,and reduce the incidence of postoperative complications.
