克氏针内固定术与PUSHLCOK内固定术治疗后交叉韧带胫骨止点撕脱骨折患者的效果比较

摘　　要：目的:探究克氏针内固定术与PUSHLCOK内固定术治疗后交叉韧带(PCL)胫骨止点撕脱骨折患者的效果。方法:选择2022年1月—2023年1月陆军第八十集团军医院骨科收治112例PCL胫骨止点撕脱骨折患者为研究对象,随机分为对照组和研究组,每组61例。对照组采用克氏针内固定术,研究组采用PUSHLCOK内固定术。对比两组围手术期指标、膝关节功能及疼痛程度、关节稳定性、并发症发生情况。结果:研究组骨折愈合时间、住院时间均短于对照组,差异均有统计学意义(P<0.05)。术后1年,研究组Lysholm膝关节评分高于对照组,疼痛数字评分法评分低于对照组,差异均有统计学意义(P<0.05)。术后1年,研究组前抽屉试验、Lachman试验阳性率分别为6.56%、11.48%,相较于对照组的8.20%、6.56%,差异均无统计学意义(P>0.05);术后1年,研究组轴移试验阳性率为9.84%,低于对照组的24.59%,差异有统计学意义(P<0.05)。研究组并发症发生率为3.28%,相较于对照组的4.92%,差异无统计学意义(P>0.05)。结论:PCL胫骨止点撕脱骨折患者采用PUSHLCOK内固定术治疗,可有效改善膝关节功能并减轻疼痛程度,利于提高关节稳定性,且具有一定的应用安全性。Objective:To explore the efficacy of Kirscher’s needle fixation and PUSHLCOK fixation in the treatment of posterior cruciate ligament(PCL)tibial insertion avulsion fracture.Methods:A total of 112 patients with PCL tibial insertion avulsion fracture admitted to the Department of Orthopedics of the 80th Group Military Hospital of the Army from January 2022 to January 2023 were selected as the study objects,and randomly divided into a control group and a study group,with 61 cases in each group.The control group underwent Kirschler needle fixation,and the study group underwent PUSHLCOK fixation.Perioperative indexes,knee function and pain degree,joint stability and complications were compared between the two groups.Results:The fracture healing time and hospital stay in the study group were shorter than those in the control group,and the differences were statistically significant(P<0.05).One year after surgery,Lysholm’s knee joint score in the study group was higher than that in the control group,the pain numerical score was lower than that in the control group,with statistical significance(P<0.05).One year after surgery,the positive rates of the front drawer test and Lachman test in the study group were 6.56%and 11.48%respectively,compared with 8.20%and 6.56%in the control group,with no statistical significance(P>0.05).One year after operation,the positive rate of axial shift test in the study group was 9.84%,which was lower than 24.59%in the control group,the difference was statistically significant(P<0.05).The incidence of complications in the study group was 3.28%,compared with 4.92%in the control group,the difference was not statistically significant(P>0.05).Conclusion:PUSHLCOK internal fixation in patients with PCL tibial arrest avulsion fracture can effectively improve knee joint function and reduce pain,improve joint stability,and has certain application safety.
