椎弓根钉内固定术治疗脊椎骨折患者的效果及对氧化应激水平的影响分析

摘　　要：目的:探讨椎弓根钉内固定术治疗脊椎骨折患者的临床效果及其对氧化应激水平的影响。方法:回顾性选择2018年9月—2023年12月核工业四一七医院收治的130例脊椎骨折患者资料作为研究对象,根据治疗方法不同将其分为椎弓根组和对照组,各65例。椎弓根组采用椎弓根钉内固定术治疗,对照组采用常规内固定术治疗。对比两组的临床疗效、腰椎功能、并发症发生情况及血清超氧化物歧化酶(SOD)、丙二醛(MDA)水平。结果:椎弓根组临床疗效的的优良率为98.46%,高于对照组的87.69%,差异有统计学意义(P<0.05)。术后3个月,两组日本骨科学会腰椎评分法评分、血清SOD水平均高于术前1 d,且椎弓根组高于对照组,两组MDA水平均低于术前1 d,且椎弓根组低于对照组,差异均有统计学意义(P<0.05)。椎弓根组并发症发生率为3.08%,低于对照组的15.38%,差异有统计学意义(P<0.05)。结论:椎椎弓根钉内固定术治疗脊椎骨折患者能提高临床疗效,改善患者的腰椎功能,调节血清SOD、MDA分泌,降低术后并发症发生率。Objective:To investigate the clinical effect of pedicle screw internal fixation in the treatment of patients with vertebral fracture and the level of oxidative stress.Methods:Data of 130 patients with vertebral fracture admitted to 417 Hospital of Nuclear Industry from September 2018 to December 2023 were retrospectively selected as the study object,and were divided into a pedicle group and a control group according to different treatment methods,with 65 cases in each group.The pedicle group was treated with pedicled screw internal fixation,the control group was treated with conventional fixation.The clinical efficacy,lumbar function,complications and serum levels of superoxide dismutase(SOD)and malondialdehyde(MDA)were compared between the two groups.Results:The excellent and good rate of clinical effect in the pedicle group was 98.46%,higher than 87.69%in the control group,the difference was statistically significant(P<0.05).Three months after surgery,the Japanese Orthopaedic Society Lumbar spine score and serum SOD level in 2 groups were higher than 1 day before surgery,and the pedicle group was higher than the control group,and the MDA level in 2 groups was lower than 1 day before surgery,and the pedicle group was lower than the control group,with statistical significance(P<0.05).The incidence of complications in the pedicle group was 3.08%,lower than 15.38%in the control group,the difference was statistically significant(P<0.05).Conclusion:Pedicled screw internal fixation can improve the clinical effect,improve the function of lumbar spine,regulate serum SOD and MDA secretion,reduce the incidence of postoperative complications.
