超声引导下针刀序贯疗法治疗肩周炎患者的效果及对肩关节活动度的影响

摘　　要：目的:探究超声引导下针刀序贯疗法治疗肩周炎患者的效果及对肩关节活动度的影响。方法:选择2021年3月—2024年3月临沂市中医医院收治的84例肩周炎患者为研究对象,按随机数字表法将其分为研究组和对照组,各42例。对照组行超声引导-痛点封闭-肩关节松动术治疗,研究组采取超声引导下针刀序贯疗法治疗,均治疗3周。对比两组疼痛程度、肩关节功能、肩关节活动度、临床疗效。结果:治疗后,研究组疼痛视觉模拟评分低于对照组,Constant-Murley肩功能评分量表评分高于对照组,肩关节内旋、外旋、后伸、外展活动度分别为(56.49±3.92)°、(63.84±5.22)°、(39.73±4.12)°、(145.33±10.09)°,均大于对照组的(47.38±3.51)°、(48.64±3.83)°、(31.55±3.88)°、(117.73±8.26)°,差异均有统计学意义(P<0.05);研究组治疗总有效率为85.71%,高于对照组的64.29%,差异有统计学意义(P<0.05)。结论:超声引导下针刀序贯疗法治疗肩周炎患者效果显著,可减轻患者疼痛,提高肩关节活动度,促进肩关节功能恢复。Objective:To explore the effect of ultrasound-guided needle knife sequential therapy on patients with periarthritis humeroscapularis and its influence on shoulder motion.Methods:A total of 84 patients with periarthritis humeroscapularis admitted to Linyi Hospital of Traditional Chinese Medicine from March 2021 to March 2024 were selected as the study objects,and were divided into a study group and a control group according to random number table method,with 42 cases in each group.The control group was treated with ultrasound guidance,pain point closure and shoulder joint loosening,while the study group was treated with ultrasound guidance and needle knife sequential therapy for 3 weeks.The pain degree,shoulder joint function,shoulder joint motion and clinical effect were compared between the two groups.Results:After treatment,the pain Visual Analogue Scale score of the study group was lower than the control group,and the Constant-Murley Shoulder Function Rating Scale score was higher than the control group,the internal rotation,external rotation,extension and abduction motion of shoulder joint were(56.49±3.92)°,(63.84±5.22)°,(39.73±4.12)°and(145.33±10.09)°,respectively,which were bigger than(47.38±3.51)°,(48.64±3.83)°,(31.55±3.88)°,(117.73±8.26)°of the control group,the differences were statistically significant(P<0.05).The total effective rate of the study group was 85.71%,which was higher than 64.29%of the control group,the difference was statistically significant(P<0.05).Conclusion:Ultrasound-guided needle knife sequential therapy is effective in the treatment of patients with periarthritis humeroscapularis,which can reduce the pain of patients,improve the mobility of shoulder joints and promote the functional recovery of shoulder joints.
