摘　　要：“阳道实,阴道虚”理论源于《黄帝内经》,该理论概括脾胃的生理病理特点,与慢性萎缩性胃炎的病因、病机关系密切,据此探讨诊治思路。在病因病机方面,脾病多虚,胃病多实。脾为阴脏,其病多虚,气血不足,胃络失养;或因虚致滞,邪滞伤胃,故“阴道虚”是发病之本。胃属阳腑,其病多实,邪阻中焦,郁而化热,蕴结成毒,导致胃黏膜萎缩甚至出现肠上皮化生等病理改变,故“阳道实”是发病的重要因素。治疗上,针对“阴道虚”宜健脾补虚;针对“阳道实”宜泻实通胃;针对阴阳失衡即胃癌前期,益胃养阴,解毒抗癌。基于“阳道实,阴道虚”理论治疗慢性萎缩性胃炎效果显著,期望对临床工作有一定启发。The theory of“Yang Dao is solid and vagina is deficient”originated from Huangdi Neijing,which summarizes the physiological and pathological characteristics of the spleen and stomach,and is closely related to the etiology and pathogenesis of chronic atrophic gastritis.In terms of etiology and pathogenesis,spleen disease is more deficient,stomach disease is more solid.The spleen is Yin and viscera,and its diseases are deficiency,insufficient Qi and blood,and dystrophy of stomach.Or because of deficiency caused by stagnation,evil stagnation hurt the stomach,so“vagina deficiency”is the root of the disease.The stomach belongs to the Yang fu-organs,and its disease is full,the evil obstructs the middle jiao,the depression and the heat,the accumulation of poison,resulting in the gastric mucosa atrophy and even the intestinal metaplasia and other pathological changes,so“Yang Dao is solid”is an important factor of the disease.Treatment,for“vagina deficiency”spleen tonifying deficiency;Aiming at“Yang Dao is solid”diarrhea and stomach;Aiming at the imbalance of Yin and Yang,that is,the early stage of gastric cancer,supplementing the stomach and nourishing Yin,detoxifying and anticancer.The treatment of chronic atrophic gastritis based on the theory of“Yang Dao is solid,vaginal deficiency”has remarkable effect,hoping to have some inspiration for clinical work.
