加味枇杷清肺饮治疗肠癌患者西妥昔单抗相关性皮疹疗效观察

摘　　要：目的:探讨加味枇杷清肺饮联合夫西地酸乳膏治疗肠癌患者西妥昔单抗所致皮疹的临床疗效,为中医药治疗肠癌患者西妥昔单抗相关性皮疹提供依据。方法:将符合纳入标准的76例肠癌患者随机分为对照组和治疗组,每组各38例。治疗期间,两组患者均接受基础治疗。对照组在此基础治疗上外用夫西地酸乳膏治疗,根据皮疹范围涂抹均匀,3次/日,而治疗组在对照组的基础上加用加味枇杷清肺饮口服治疗,200 mL/次,3次/日,7天为1疗程,持续治疗4个疗程。比较两组患者治疗前后皮疹分级、中医证候积分以及皮肤病生活质量评分。结果:治疗组皮疹疗效总有效率略高于对照组,两者之间差异无统计学意义(P>0.05)。治疗组皮疹分级改善优于对照组(P<0.05)。治疗组中医证候总有效率(84.21%)高于对照组(65.79%),两者之间差异具有统计学意义(P<0.05)。治疗组的中医证候总积分降低幅度显著高于对照组(P<0.05),在瘙痒与疼痛、口干的积分可见明显改善(P<0.05)。两组患者生活质量评分较治疗前均有所下降,但两组之间差异无统计学意义(P>0.05)。结论:加味枇杷清肺饮联合外用夫西地酸乳膏能明显改善西妥昔单抗相关性皮疹的症状,提高患者生活质量。Objective:To investigate the clinical effectiveness of Jiawei Pipa Qingfei Decoction in combination with fusidic acid cream for managing cetuximab-induced rash in colorectal cancer patients,and to establish a foundation for the treatment of rituximab-induced rashes in patients with colorectal cancer using traditional Chinese medicine.Methods:76 colorectal cancer patients meeting inclusion criteria were randomly assigned to the control or treatment group,with each group comprising 38 cases.Both groups received standardized primary therapy.The control group applied fusidic acid cream three times a day,while the treatment group received Jiawei Pipa Qingfei Decoction in addition to the control group's treatment,with a dosage of 200ml per administration three times a day,for a total of seven days per treatment course,and a cumulative total of four courses.The study conducted a comparative analysis of rash classification,traditional Chinese medicine syndrome scores,and skin disease quality of life scores before and after treatment in both groups.Results:The treatment group showed a slightly higher total response rate in rash efficacy compared to the control group,though not statistically significant(P>0.05).The improvement in rash grade was significantly more pronounced in the treatment group(P<0.05).The total effective rate of traditional Chinese medicine syndrome in the treatment group(84.21%)surpassed the control group(65.79%)significantly indicating a significant difference between the two groups(P<0.05).Traditional Chinese medicine syndrome scores in the treatment group were significantly lower than the control group(P<0.05),particularly in dry mouth,itching and pain scores(P<0.05).Both groups exhibited decreased quality of life scores post-treatment,with no statistically significant difference between them(P>0.05).Conclusion:The combination of Jiawei Pipa Qingfei Decoction and fusidic acid cream significantly improves cetuximab-induced rash symptoms and enhances the overall quality of life in patients with colore
