张钟爱教授治疗慢性胃炎经验及组方用药规律分析

摘　　要：目的:基于“中医传承辅助平台系统(V3.0)”探讨张钟爱教授治疗慢性胃炎的用药规律,为临床慢性胃炎的治疗提供参考。方法:收集整理张钟爱教授于南京市中医院金陵名医馆门诊治疗慢性胃炎的经验处方,将符合标准的278张中药处方依次录于中医传承辅助平台V3.0,运用频次统计、关联规则分析、改进互信息法等方法进行用药规律及特点挖掘,探索并分析治疗慢性胃炎用药规律。结果:张钟爱教授治疗慢性胃炎的278张处方中,用药频率排名前九的为陈皮(253)、半夏(241)、茯苓(227)、黄连(220)、竹茹(202)、白及(193)、瓦楞子(168)、枳壳(147)、刀豆壳(141);所用药物四气多为温、寒、平,五味多为苦、甘、辛,主归肺、脾、胃、肝、心经;药物功效排名前5的为理气类(999)、清热类(570)、化痰类(547)、补虚类(479)、利水渗湿类(437);常用的药物组合为陈皮-半夏、陈皮-茯苓、半夏-茯苓、陈皮-半夏-茯苓等。高频药物聚类得到5个聚类组。关联分析得出治疗核心方1个:黄连、陈皮、半夏、茯苓、白及、竹茹、甘草;得出新方5个。结论:张钟爱教授治疗慢性胃炎,用药轻清,以“理气健脾化湿”为主,擅用理气化痰、健脾和胃、清热化湿之药,其治疗具有清补兼施、中正平和、因势利导的特点。Objective:To explore the medication rule of Professor Zhang Zhongai in the treatment of chronic gastritis based on traditional Chinese Medicine inheritance Auxiliary platform system(V3.0),so as to provide reference for clinical treatment of chronic gastritis.Methods:Collect and organize sheets Professor Zhang zhongai's experience prescription for the treatment of chronic gastritis in the outpatient clinic of Jinling Famous Medical Center of Nanjing Hospital of Traditional Chinese Medicine 278 TCM prescriptions that met the criteria were sequentially recorded on the TCM Inheritance Auxiliary Platform V3.0.Methods such as association rule analysis and improved mutual information method were used to explore the rules and characteristics of drug use The law of medication for the treatment of chronic gastritis was analyzed.Results:Among Professor Zhang Zhongai's 278 prescriptions for chronic gastritis,the top nine were tangerine peel(253),Pinellia ternata(241),Poria(227),Coptis chinensis(220),Zhu Ru(202),Bai Ji(193),Corrugated fruit(168),Fructus Aurantii(147),and Knife bean shell(141).Most of the four qi used were warm,cold,flat,and the five flavors were bitter,sweet and pungent,mainly to the lung,spleen,stomach,liver and heart meridian.The top five drugs are regulating qi(999),clearing heat(570),resolving phlegm(547),tonifying deficiency(479),diuresis and dampness(437).The commonly used drug combinations are:Tangerine peel-Pinellia ternata,Tangerine peel-Poria cocos,Pinellia ternata-Poria cocos,Tangerine peel-Pinellia ternata-Poria cocos and so on.Five cluster groups were obtained by high frequency drug clustering.Correlation analysis showed that the core prescriptions of treatment were Coptis chinensis,Tangerine peel,Pinellia ternata,Poria cocos,Bai Ji,Zhu Ru and licorice,and 5 new prescriptions were obtained.Conclusion:Professor Zhang Zhongai treats chronic gastritis with light clearing,mainly“regulating qi,invigorating spleen and resolving dampness”,and is good at using the drugs of regulating qi and resolving
