石强教授用虫类药辨治肺系疾病数据研究

摘　　要：目的:探讨石强教授用虫类药辨治肺系疾病的经验。方法:搜集石强教授用虫类药治疗肺系疾病的医案,将医案中的四诊信息、证候结论、虫类药名称等进行规范化处理,运用Microsoft Excel 2021进行频数统计、SAS 9.4进行关联规则分析和Cytoscape 3.8.0构建“虫类药-虫类药”关联规则网络图。结果:共收集有效医案292份,涉及虫类药24种。频数统计显示四诊信息以咳嗽为主;肺系疾病病因以热邪最高、病位以太阴为主、病机以郁为多;高频虫类药为蝉蜕、乌梢蛇、蕲蛇等,虫类药四气以寒为主,五味以甘、咸为多,归经主入肝经,毒性虫类药仅有4味。关联分析得出四诊信息与肺系疾病病因、病位、病机,虫类药与肺系疾病病因、病位、病机,以及虫类药之间的关联关系。结论:虫类药在治疗肺系疾病中可发挥疏外风、祛伏风、和肝脾、调升降和搜剔通络五大功效。蝉蜕可治疗肺系疾病所表现出的咽痒咳、鼻痒、流清涕、喷嚏、苔薄、两寸浮等外风症状及体征;乌梢蛇、蜂房、全蝎、蜈蚣、蕲蛇等可治肺系疾病所表现出的天突痒咳、越咳越痒、鼻根两侧青、夜寐动惕、脉浮弦等伏风症状及体征;蝉蜕、乌梢蛇、蕲蛇、蜂房等可治肺系疾病所表现出的便前腹痛、大便溏、流涎、夜寐俯卧、苔滑等肝脾不调症状及体征;蝉蜕配乌梢蛇、蝉蜕配蕲蛇等虫类药药对可治疗肺系疾病所表现出的连声咳、气上冲咳、运动后咳、苔白底浮黄、脉弦涩等升降失常症状及体征;地龙可用于治疗肺系疾病所表现出的气憋、舌下络脉紫暗等久病入络症状及体征。Objective:To explore Professor Shi Qiang's clinical experience in treating pulmonary diseases with medicinal insects.Methods:The medical records of Professor Shi Qiang for the treatment of pulmonary diseases with medicinal insects were collected.And the words in the medical records,such as the four diagnostic information,the conclusion of syndromes,and the name of medicinal insects,were standardized.Microsoft Excel 2021 was used to do frequency statistics.SAS 9.4 was used to do the association rule analysis.Cytoscape 3.8.0 was used to construct the network figure of the association rule between two medicinal insects.Results:292 effective medical records were collected,involving 24 kinds of medicinal insects.The frequency statistics showed that cough accounted for the largest proportion in the four diagnostic information;the heat evil had the highest frequency in the etiology of pulmonary diseases;Taiyin was the main one in the location of pulmonary diseases;the constraint is the main factor in the pathogenesis of pulmonary diseases.The high-frequency medicinal insects were Cicada slough,Zaocys dhumnade,Agkistrodon acutus,etc.The four natures of medicinal insects is mainly cold,the five tastes are mainly sweet and salty,and the meridian tropism is mainly liver channel.There are four medicinal insects has poisonous.The association between the four diagnostic information and the etiology,location and pathogenesis of pulmonary diseases,and the association between the medicinal insects and the etiology,location and pathogenesis of pulmonary diseases,as well as between two medicinal insects were obtained by the association rule analysis.Conclusion:The medicinal insects can exert five functions in the treatment of pulmonary diseases,including relieving external wind,dispelling latent wind,harmonizing the liver and spleen,adjusting up and down,and searching and dredging collaterals.Cicada slough can treat symptoms and signs of external wind such as throat itching,nasal itching,runny nose,sneezing,thin fur,and floating pu
