摘 要:近年来冠状动脉粥样硬化发病率不断升高,急性心肌梗死是其最严重的发病形式。目前,临床治疗的主要目的在于尽早开通冠脉循环,恢复心肌血供。微循环栓塞是治疗急性心肌梗死的关键,尽早开通微循环对于提升急性心肌梗死的治疗质量至关重要。超声声学溶栓疗法能够有效治疗包括微循环栓塞在内的血管内血栓栓塞,且在获得良好溶栓治疗效果的同时,有望实现如冠状动脉微血管血栓的清除作用及终末循环功能的恢复,对于冠状动脉血栓栓塞治疗具有潜在的临床应用价值。基于此,本文就超声声学溶栓疗法在冠状动脉血栓栓塞治疗中的应用探索、治疗作用机制及其研究进展进行综述。The incidence of coronary atherosclerosis has been increasing in recent years,with acute myocardial infarction as its most severe onset.The main clinical treatment aim is to restore myocardial blood supply by recanalizing coronary circulation as early as possible.The existing issue of microcirculation thromboembolism has become an obstacle.Thrombus elimination in coronary microcirculation is crucial and essential to improve the treatment effect of acute myocardial infarction.In recent years,sonothrombolysis has effectively solved the problem of vascular thromboembolism,including microcirculation thromboembolism,and the treatment method is expected to obtain satisfied thrombolytic treatment effect with microthrombus elimination in coronary microvessels and function recovery of terminal microcirculation,which has potential clinical value for the establishment of novel treatment method for coronary thromboembolism.Thus,this paper reviews sonothrombolysis for the application exploration in the treatment of coronary artery thromboembolism,the mechanism of treatment action,and its research progress.
