摘　　要：目的:从处方信息中识别风险信号进行药物滥用筛查,建立基于此方法的药物滥用主动监测模式。方法:提取2020—2022年门诊精二药品处方信息,根据“超剂量”“超疗程”“精二药品联合使用”“替代治疗”“戒断用药”和“自行加量”六项指标筛查疑似药物滥用患者,并使用标准化访谈提纲进行药物依赖评定。根据评定结果将疑似人群分为依赖组和非依赖组,分析比较六项风险信号的筛查效果。结果:筛查共发现995名疑似药物滥用患者,其中167人评定为药物滥用。两组间交叉分析结果显示,超剂量、精二联用、替代治疗、戒断用药和自行加量在筛查效果方面有显著差异(P<0.05)。根据筛查结果分析比较情况建立药物滥用监测触发词库和排除条件库。结论:从处方信息中提取风险信号作为筛查工具的药物滥用主动监测方法可行,值得进一步研究推广。Objective:To identify risk signals from prescription information for drug abuse screening,and to establish an active drug abuse monitoring model based on this method.Methods:The prescription information of class Ⅱ psychotropic drugs in outpatient department from 2020 to 2022 was extracted.Suspected drug abuse patients were screened according to six indicators:overdos,over-treatment,combined use of second-class spermine drugs,alternative therapy,withdrawal medication and self-increment,and drug dependence was assessed using standardized interview outline.According to the evaluation results,the suspected population was divided into dependent group and non-dependent group,and the screening effects of six risk signals were analyzed and compared.Results:A total of 995 suspected drug dependent patients were found during screening,167 of them were rated as drug abuse.The results of cross analysis between the two groups showed that overdose,combined use of psychotropic drugs,replacement therapy,withdrawal therapy and self-increment showed significant differences in screening effectiveness(P<0.05).Established a database of trigger words and exclusion conditions for drug abuse monitoring based on screening results analysis and comparison.Conclusion:Extracting risk signals from prescription information as a screening tool for active drug abuse monitoring is feasible.This model is worth further research and promotion.
