摘 要:目的:研究不同浓度罗哌卡因复合或不复合右美托咪定在上肢手术中的麻醉效果及安全性。方法:选择我院2021年1月—2022年12月收治的80例上肢手术患者,以麻醉方式差异分为四组,分别为A组(单纯低浓度罗哌卡因)、B组(低浓度罗哌卡因复合右美托咪定)、C组(单纯高浓度罗哌卡因)、D组(高浓度罗哌卡因复合右美托咪定),各20例。比较各组麻醉效果、术中血流动力学及术后疼痛评分。结果:D组麻醉起效时间短于其他组,感觉、运动阻滞时间长于其他组,肌皮神经、桡神经和尺神经完全阻滞率高于其他组(P<0.05);D组手术结束时平均动脉压高于其他组(P<0.05);D组术后苏醒时、术后1h静息和咳嗽时疼痛评分低于其他组(P<0.05)。结论:高浓度罗哌卡因复合右美托咪定可提高上肢手术中的麻醉效果,对术中血流动力学影响较小,可减轻疼痛感受,麻醉安全性可。Objective:To study the anesthetic effect and safety of different concentrations of ropivacaine combined with or without dexmedetomidine in upper limb surgery.Methods:A total of 80 patients with upper limb surgery admitted to our hospital from January 2021 to December 2022 were divided into four groups according to different anesthesia.Group A(simple low concentration ropivacaine),group B(low concentration ropivacaine combined with dexmedetomidine),group C(simple high concentration ropivacaine combined with dexmedetomidine),with 20 patients in each group.The anesthetic effect,intraoperative hemodynamics and postoperative pain score were compared among different groups.Results:The onset time of anesthesia in group D was shorter than that in other groups,the time of sensory and motor block was longer than that in other groups,and the complete block rate of musculocutaneous nerve,radial nerve and ulnar nerve was higher than that in other groups(P<0.05).The mean arterial pressure in group D was higher than that in other groups at the end of operation(P<0.05).The pain scores of awakening,resting and coughing at 1 h after operation in group D were lower than those in other groups(P<0.05).Conclusion:High concentration ropivacaine combined with dexmedetomidine can improve the anesthesia effect in upper limb surgery,have little effect on intraoperative hemodynamics,reduce pain perception,and have good anesthesia safety.
