摘　　要：目的:关于2022—2023年我院肿瘤科Ⅱ类~Ⅲ类手术切口围术期预防性应用抗菌药物合理性调查分析。方法:回顾性提取2022年6月—2023年12月于信阳市中心医院肿瘤科1476例接受手术治疗患者的归档病历,对其抗菌药物的使用情况进行整合分析,并评估抗菌药物应用的合理性。结果:1476例患者病历中,单用1种抗菌药物的有1378例,联合2种及以上的有98例;抗菌药物共涉及7个类别,共23个品种,其中青霉素类1种、头孢类11种、喹诺酮类2种、含β内酰胺类抗生素的复方制剂3种、硝基咪唑类2种、大环内酯类1种、其他类2种;术前0.5 h给药共有1390例,术前0.5~1 h给药61例,术后给药25例;用药时间≤48 h 1408例,>48 h 68例;84例患者存在不合理预防性应用抗菌药物情况,依次为用药疗程过长、抗菌用药品种不适宜、给药时机不合理、给药剂量不合理、给药途径不适宜、联合用药不合理。结论:本院肿瘤科尚存在用药疗程过长、用药品种不适宜、给药时机不合理等问题,需要加强医务人员规范应用抗菌药物的知识培训,并健全合理应用抗菌药物的监督体系。Objective:To investigate the rationality of prophylactic antibiotics in the oncology department of our hospital from 2022 to 2023.Methods:The archived medical records of 1476 patients receiving surgical treatment in the Oncology Department of Xinyang Central Hospital from June 2022 to December 2023 were retrospectively extracted,and the use of antibacterial drugs was analyzed and the rationality of antibacterial application was evaluated.Results:Among the 1476 patients,1378 cases were treated with only one kind of antibacterial drug,and 98 cases were treated with two or more kinds of antibacterial drugs.A total of 7 categories and 23 varieties of antibiotics were involved,including 1 kind of penicillins,11 kinds of cephalosporins,2 kinds of quinolones,3 kinds of compound preparations containing β-lactam antibiotics,2 kinds of nitroimidazoles,1 kind of macrolides and 2 kinds of others.A total of 1390 cases were administered 0.5 h before operation,61 cases were administered 0.5-1 h before operation,and 25 cases were administered after operation.There were 1408 cases with medication time≤48 h and 68 cases with medication time>48 h.84 patients had unreasonable prophylactic use of antibiotics,followed by long course of treatment,inappropriate variety of antibiotics,unreasonable timing of administration,unreasonable dosage,inappropriate route of administration,and unreasonable combination of drugs.Conclusion:There are still some problems in the oncology department of our hospital,such as long course of treatment,inappropriate drug varieties and unreasonable timing of administration.It is necessary to strengthen the knowledge training of medical staff in the standardized application of antibiotics and improve the supervision system of rational application of antibiotics.
