益肾补骨汤联合阿法骨化醇治疗骨质疏松性骨折椎体成形术后患者的效果

摘　　要：目的:探讨益肾补骨汤联合阿法骨化醇治疗腰椎骨质疏松性骨折椎体成形术后的疗效及对血清骨钙素(BGP)、胶原交联氨基肽末端肽(NTX)、胶原交联C末端肽(CTX)的影响。方法:选择2020年1月—2021年12月我院收治的80例腰椎骨质疏松性骨折椎体成形术患者,随机分为对照组和实验组,各40例。对照组术后使用阿法骨化醇治疗,实验组在对照组基础上加用益肾补骨汤治疗。比较两组治疗后腰背痛VAS评分、骨折愈合时间、JOA评分、骨密度(BMD)、血清BGP、CTX、NTX水平以及术后再发骨折情况。结果:实验组用药1周VAS评分低于对照组(P<0.05),而两组用药4周、8周、12周VAS评分比较,差异无统计学意义(P>0.05)。实验组骨折愈合时间短于对照组(P<0.05),而两组JOA评分比较,差异无统计学意义(P>0.05)。实验组腰椎L2~4、股骨颈BMD高于对照组(P<0.05)。实验组血清BGP水平高于对照组(P<0.05),而两组NTX、CTX水平比较,差异无统计学意义(P>0.05)。两组术后再发骨折率比较,差异无统计学意义(P>0.05)。结论:腰椎骨质疏松性骨折椎体成形术后患者使用益肾补骨汤联合阿法骨化醇治疗有利于缓解早期疼痛,促进骨折愈合,并改善骨转换标志物,提高骨密度。Objective:To investigate the efficacy of Yishen Bugu Decoction combined with alfacalcidol in the treatment of lumbar osteoporotic fracture after vertebroplasty and its effect on serum osteocalcin(BGP),collagen cross-linked aminopeptide terminal peptide(NTX)and collagen cross-linked C-terminal peptide(CTX).Methods:From January 2020 to December 2021,80 patients with lumbar osteoporotic fracture treated by vertebroplasty in our hospital were selected and randomly divided into control group and experimental group,with 40 patients in each group.The control group was treated with alfacalcidol after operation,and the experimental group was treated with Yishen Bugu Decoction on the basis of the control group.The VAS score of low back pain,fracture healing time,JOA score,bone mineral density(BMD),serum BGP,CTX,NTX levels and postoperative recurrence of fractures were compared between the two groups.Results:The VAS score of the experimental group was lower than that of the control group(P<0.05),while there was no significant difference in VAS scores between the two groups at 4 weeks,8 weeks and 12 weeks(P>0.05).The fracture healing time of the experimental group was shorter than that of the control group(P<0.05),while there was no significant difference in JOA score between the two groups(P>0.05).The BMD of lumbar spine L_(2-4)and femoral neck in the experimental group was higher than that in the control group(P<0.05).The serum BGP level in the experimental group was higher than that in the control group(P<0.05),but there was no significant difference in the levels of NTX and CTX between the two groups(P>0.05).There was no significant difference in the rate of recurrent fracture between the two groups(P>0.05).Conclusion:The treatment of Yishen Bugu Decoction combined with alfacalcidol in patients with lumbar osteoporotic fracture after vertebroplasty is beneficial to relieve early pain,promote fracture healing,improve bone turnover markers and increase bone mineral density.
