基于“既病防变”理论探析清肺排毒汤防治COVID-19相关性急性肾损伤

摘　　要：在新型冠状病毒感染危重患者中,出现急性肾损伤的情况并不罕见。清肺排毒汤作为通用方被列为新型冠状病毒肺炎(COVID-19)中医临床治疗首选,现有证据表明,其可防治与COVID-19相关的急性肾损伤。本文从“既病防变”的角度出发,探讨清肺排毒汤防治COVID-19相关性急性肾损伤的证治规律。本文还基于中医学中的五行学说、三焦辨证和六经传变等相关理论,提出“泻母补子”“三焦分治”和“太少两解”的“既病防变”新思路,以期为中医临床应对疫病防治提供借鉴。Acute kidney injury(AKI)is not uncommon in critically ill patients infected with Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2).Qingfei Paidu Decoction,as a general prescription,has been listed as the first choice for the clinical treatment of coronavirus disease 2019(COVID-19)in Traditional Chinese medicine.Current evidence suggests that it can prevent and treat COVID-19-related AKI.This article explored the law of syndrome differentiation and treatment of Qingfei Paidu Decoction in preventing and treating acute renal injury related to COVID-19 from the perspective of“preventing disease from exacerbating”.And this article put forward a new idea of“Preventing Disease from Exacerbating”by“Purging Mother and Tonifying Son”,“Treating Triple Energizer Separately”,and“Treating Taiyang and Shaoyin Together”based on the theory of five elements,syndrome differentiation of triple energizer,and the transmission of six meridians in traditional Chinese medicine,so as to provide better reference for clinical prevention and treatment of epidemic diseases in traditional Chinese medicine.
