摘　　要：目的探讨类风湿关节炎(RA)患者血清胰岛素样生长因子ⅡmRNA结合蛋白3(IGF2BP3)水平及其临床意义。方法选取并收集2019年12月至2022年2月温州医科大学附属东阳医院收治的RA患者82例血清作为RA患者组,健康对照人群45名来自同期体检中心作为对照组。采用ELISA法测定2组血清IGF2BP3水平,t检验比较2组间IGF2BP3水平差异。通过绘制受试者工作特征曲线(ROC)分析IGF2BP3在RA诊断中的价值,确定截断值后将血清IGF2BP3分成高水平和低水平2组,采用χ^(2)检验分析血清IGF2BP3水平与RA患者各项临床特征的关系。结果RA患者血清IGF2BP3水平[(0.60±0.55)ng/ml]显著高于对照组[(0.31±0.17)ng/ml],差异有统计学意义(t=4.42,P<0.001)。根据ROC曲线确定0.308 ng/ml为血清IGF2BP3的截断值。活动期RA患者血清IGF2BP3高水平率(78.8%,41/52)显著高于非活动期患者(56.7%,17/30)(χ^(2)=4.52,P=0.033)。RA患者血清IGF2BP3水平与CRP水平呈正相关(r=0.29,P=0.008),CRP高水平组患者血清IGF2BP3高水平率为82.0%(41/50),显著高于CRP低水平组(54.8%,17/31)(χ^(2)=6.94,P=0.008)。进一步分析血清IGF2BP3诊断RA的价值,显示曲线下面积(AUC)为0.700,灵敏度为70.7%,特异度为60.0%。另取0.575、0.689、0.727 ng/ml作为血清IGF2BP3的截断值时,其灵敏度分别为34.1%、26.8%、23.2%;特异度分别为95.6%、97.8%、100%。结论血清IGF2BP3水平与RA患者疾病进展相关。Objective To investigate the levels and clinical significance of insulin-like growth factor 2 mRNA binding protein 3(IGF2BP3)in the serum of patients with rheumatoid arthritis(RA).Methods Serum samples from 82 patients with RA admitted to the Affiliated Dongyang Hospital to wenzhou Medical Umiversity from December 2019 to February 2022 were collected as the experimental group and the serum of 45 healthy individuals from the physical examination center as the control group.Serum levels of IGF2BP3 were measured using enzyme-linked immunosorbent assay(ELISA),and the t-test was used to compare the difference in IGF2BP3 levels between the two groups.The value of IGF2BP3 in the diagnosis of RA was analyzed by plotting a ROC curve,and after determining the cut-off value,serum IGF2BP3 levels were divided into high and low levels.The relationship between serum IGF2BP3 levels and various clinical features of RA patients was then analyzed using the chi-square test.Results The serum levels of IGF2BP3 in RA patients were significantly higher compared to healthy controls[(0.60±0.55)ng/ml vs.(0.31±0.17)ng/ml,t=4.42,P<0.001].The cut-off value of IGF2BP3 was determined as 0.308 ng/ml.The high level of IGF2BP3 was significantly more prevalent in active RA patients(78.8%,41/52)than in non-active patients(56.7%,17/30)(χ^(2)=4.52,P=0.033).The level of IGF2BP3 was positively correlated with the C-reactive protein level(r=0.29,P=0.008),and the prevalence of high IGF2BP3 level in the elevated CRP group(82.0%,41/50)was significantly higher than that in the decreased CRP group(54.8%,17/31)(χ^(2)=6.94,P=0.008).The AUC of the ROC curve for serum IGF2BP3 in the diagnosis of RA was 0.700,with a sensitivity of 70.7%and a specificity of 60.0%.When using 0.575,0.689,and 0.727 ng/ml as the cut-off values for serum IGF2BP3,the sensitivity was 34.1%,26.8%,and 23.2%,respectively,with the specificity of 95.6%,97.8%,and 100.0%,respectively.Conclusion The level of serum IGF2BP3 is closely associated with RA disease progression.
