军事飞行人员体检心电图ST-T改变航空医学鉴定分析

摘　　要：目的探讨军事飞行人员心电图ST-T改变的诊治和航空医学鉴定原则。方法对2023年2月至2023年11月在海军青岛特勤疗养中心参加年度大体检且检出心电图ST-T改变的27例军事飞行人员进行心得安试验和(或)平板运动试验检查,结合病史调查、血生化和心脏超声等检查结果进行航空医学鉴定。结果27例ST-T改变军事飞行人员中,15例心率高于60次/min,行心得安试验,其中12例阳性,3例阴性;12例心率低于60次/min的飞行人员以及3例心得安试验阴性的飞行人员行平板运动试验,结果均为阴性。27例均无临床症状,结合病史调查、心脏超声等检查,排除器质性心脏病,航空医学鉴定结论均为飞行合格。结论对于飞行人员心电图ST-T非特异性改变,需结合临床,经过心得安试验、24 h动态心电图、平板运动试验、冠状动脉CT血管成像或冠状动脉造影等相关检查,依据疾病严重程度和治疗效果进行航空医学鉴定。ObjectiveTo explore the diagnosis and treatment of ST-T changes in ECG of military flying personnel and the principles of aeromedical assessment.MethodsThe propranolol test and(or)the treadmill exercise test were conducted on 27 military flying personnel who participated in the annual physical examination in Qingdao Special Service Sanatorium Center of Navy from February of 2023 to November of 2023 and tested the ST-T changes of EEG.The aeromedical assessment was made based on medical history investigation,blood biochemistry analysis,cardiac ultrasound and other examination results.ResultsAmong 27 ST-T changed military flying personnel,15 cases were with higher heart rate than 60 beats/min,and were done propranolol test.Among them 12 cases were positive and 3 cases were negative in propranolol test.Other 12 cases were with lower heart rate than 60 beats/min and 3 negative cases in propranolol test were conducted treadmill test.All of them showed negative in treadmill exercise.All 27 cases were asymptomatic.Based on medical history investigation,ECG and other examinations,organic heart disease was ruled out.The aeromedical assessment was concluded as qualified for flight.ConclusionsFor the non-specific changes of ST-T in ECG of flying personnel,it is necessary to conduct aeromedical assessment based on a series of related examinations such as propranolol test,24 h dynamic ECG,treadmill exercise test,coronary CT or coronary angiography,as well as the severity of the disease and the effect of treatment.
