摘 要:目的分析不同病程糖尿病患者周围神经病变(DPN)电生理特点。方法收集2023年3月至2024年3月山西省汾阳医院收治的200例DPN患者为研究对象。根据患者糖尿病病程不同分组,分为<1年组(36例)、1~10年组(100例)和>10年组(64例)。所有患者进行肌电图检查,比较各组患者电生理异常情况。结果<1年组肌电图异常率为2.0%,1~10年组异常率为4.0%,>10年组异常率为13.0%,3组患者肌电图异常率比较,差异具有统计学意义(P<0.05)。<1年组正中神经、尺神经、腓总神经和胫神经运动神经传导速度、复合肌肉动作电位波幅均较1~10年组和>10年组高,1~10年组较>10年组高(P<0.05);<1年组各神经远端运动潜伏期均低于1~10年组和>10年组,1~10年组较>10年组低(P<0.05)。<1年组正中神经、尺神经和腓总神经感觉神经传导速度和感觉神经动作电位波幅较1~10年组和>10年组高,1~10年组以上指标较>10年组高(P<0.05)。<1年组F波和H反射异常率均较1~10年组和>10年组低,1~10年组异常率较>10年组低(P<0.05)。结论糖尿病患者病程越长,电生理异常率越高。Objective To analyze the electrophysiological characteristics of peripheral neuropathy in diabetic patients with different course of disease.Methods A total of 200 patients with diabetic peripheral neuropathy(DPN)admitted to Shanxi Province Fenyang Hospital from March 2023 to March 2024 were collected as the study objects.The patients were divided into<1 year group(36 cases),1-10 years group(100 cases)and>10 years group(64 cases)according to the course of diabetes.All patients were examined by electromyography,and the electrophysiological abnormalities were compared among the three groups.Results The abnormal rate of electromyography was 2.0%in the<1 year group,4.0%in the 1-10 year group,and 13.0%in the>10 year group,and the difference was statistically significant(P<0.05).The motor nerve conduction velocity and composite muscle action potential amplitude of median nerve,ulnar nerve,common peroneal nerve and tibial nerve in the<1 year group were higher than those in the 1-10 year group and>10 year group,and those in the 1-10 year group were higher than those in the>10 year group(all P<0.05).The distal motor latency in the<1 year group was lower than that in the 1-10 year group and>10 year group,and that in the 1-10 year group was lower than that in the>10 year group(P<0.05).The sensory nerve conduction velocity and sensory neural action potential amplitude of median nerve,ulnar nerve,common peroneal nerve were higher in the<1 year group than those in the 1-10 years group and>10 years group,and those in the 1-10 years group were higher than those in the>10 years group(P<0.05).The abnormal rates of F wave and H reflection were lower in the<1 year group than those in the 1-10 years group and>10 years group,and the abnormal rates were lower in the 1-10 years group than those in the>10 years group(P<0.05).Conclusion The longer the course of diabetes,the higher the electrophysiological abnormality rate.
