摘  要:本文报道2例以双下肢无力为首发症状的甲状腺功能亢进性周期性麻痹患者,分析其临床特点及神经电生理检查。2例患者长时程运动诱发试验均为阳性,第1例患者于运动后60 min小指展肌复合肌肉动作电位波幅降低率为42.11%,第2例患者于运动后40 min小指展肌复合肌肉动作电位波幅降低率为58.70%。长时程运动诱发试验对发作间歇期的周期性麻痹患者是一种重要的辅助检查,具有相对特异性,当临床遇到以双下肢无力为首发表现的患者时,应仔细询问病史,及早完善甲状腺功能检查、血钾及神经电生理检查,减少该病的漏诊率及误诊率,以免延误患者的病情。This article reported two cases of hyperthyroidism periodic paralysis patients with weakness in both lower limbs as the initial symptom,and analyzed their clinical characteristics and neuroelectrophysiological examination.Two patients showed positive results in long-term exercise-induced tests.The first patient had a reduction rate of 42.11%in amplitude of composite muscle action potential of abductor little finger at 60 minutes after exercise,while the second patient had a reduction rate of 58.70%at 40 minutes after exercise.Long-term exercise-induced test is an important auxiliary examination for patients with periodic paralysis during intermittent attacks,which is relatively specific.When patients with weakness of both lower limbs are encountered in clinical practice,it is necessary to carefully inquire about the medical history,and thyroid function examination,blood potassium and nerve electrophysiological examination should be performed as soon as possible to reduce the rate of missed diagnosis and misdiagnosis of the disease,so as to avoid delaying the patient's condition.
