摘　　要：【目的】探讨老年心力衰竭(简称心衰)患者实施微型营养评价(MNA)理念4I营养支持护理的效果。【方法】本院收治的96例老年心衰患者,随机分为对照组及观察组,每组48例。对照组实施常规护理,观察组实施MNA理念4I营养支持护理。比较两组干预前后相关指标(包括白细胞水平、尿pH值、Na+水平、血糖、总胆固醇、体重等)、营养支持情况[主观整体营养评估量表(PG-SGA)评分]、心功能指标[左室射血分数(LVEF)、脑钠肽(BNP)、N-末端B型脑钠肽前体(NT-proBNP)]。【结果】干预后,观察组白细胞计数、尿pH值及Na+水平高于对照组,血糖、总胆固醇及体重低于对照组(P<0.05)。观察组PG-SGA评分、LVEF高于对照组,BNP、NT-proBNP水平低于对照组(P<0.05)。【结论】对老年心衰患者实施MNA理念4I营养支持护理,可改善患者机体状况及营养支持效果。【Objective】To explore the effects of implementing MNA(mini nutritional assessment)concept 4I(identify,intervene,inspect,improve)nutritional support nursing in elderly heart failure patients.【Methods】A total of 96 elderly heart failure patients were randomly divided into a control group and an observation group,with 48 patients in each group.The control group received routine nursing care,while the observation group received MNA concept 4I nutritional support nursing.The relevant indicators(including white blood cell level,urine pH,Na+level,blood glucose,total cholesterol,and weight)were compared before and after the intervention,along with nutritional support status[measured by the Patient-generated Subjective Global Assessment(PG-SGA)score]and cardiac function[left ventricular ejection fraction(LVEF),brain natriuretic peptide(BNP),and N-terminal pro B type natriuretic peptide(NT-proBNP)].【Results】After the intervention,the observation group showed higher level of white blood cell,urine pH,and Na+,and lower level of blood glucose,total cholesterol,and weight compared to the control group(P<0.05).The PG-SGA score and LVEF level in the observation group were higher,while BNP and NT-proBNP levels were lower than those in the control group(P<0.05).【Conclusion】Implementing MNA concept 4I nutritional support nursing in elderly heart failure patients can improve the patients'physical condition and nutritional support effectiveness.
