摘　　要：【目的】探讨司维拉姆联合血液灌流疗法对尿毒症皮肤瘙痒患者瘙痒程度评分及钙、磷代谢指标的影响。【方法】选取2022年7月至2024年2月在本院诊治的76例尿毒症皮肤瘙痒患者,按照随机数字表法分为对照组(给予血液灌流疗法)和观察组(在对照组基础上给予司维拉姆治疗),每组38例。比较两组瘙痒程度评分、生存质量评分、炎症因子水平及钙、磷代谢指标水平。【结果】治疗后,观察组睡眠、症状和体征、社会心理评分及超敏C反应蛋白(hs-CRP)、降钙素原(PCT)、白细胞介素6(IL-6)、血磷、血钙、全段甲状旁腺素(iPTH)水平均低于对照组,世界卫生组织生存质量测定量表(WHOQOL-BREF)各项评分均高于对照组,差异均有统计学意义(P<0.05)。【结论】采用司维拉姆联合血液灌流疗法治疗尿毒症皮肤瘙痒患者的效果显著,可缓解瘙痒症状,减轻机体炎症反应,改善钙、磷代谢指标,提高患者生存质量。【Objective】To explore the effect of sevelamer combined with hemoperfusion therapy on itching severity and calcium-phosphorus metabolism in patients with uremic skin itching.【Methods】A total of 76 patients with uremic skin itching admitted to the hospital from July 2022 to February 2024 were randomly divided into two groups using a random number table.The control group(n=38 cases)received blood perfusion therapy,while the observation group(n=38 cases)received sevelamer in addition to hemoperfusion of the control group.The itch severity score,inflammatory factors,calcium and phosphorus metabolism indicator,and quality of life were compared between two groups.【Results】After treatment,the sleep score,symptom and sign score,social psychological score,high-sensitivity C-reactive protein(hs-CRP),procalcitonin(PCT),interleukin-6(IL-6),blood phosphorus,blood calcium,and total parathyroid hormone(iPTH)levels in the observation group were all lower than those in the control group(P<0.05).The World Health Organization Quality of Life Inventory(WHOQOL-BREF)scores were higher than those in the control group,with significantly statistical differences(P<0.05).【Conclusion】The application of sevelamer combined with blood perfusion therapy has significant therapeutic effects,which can alleviate itching symptoms,reduce inflammatory reactions,improve calcium and phosphorus metabolism indicators,and enhance quality of life.
