摘 要:目的比较窄带成像加放大内镜与白光内镜在儿童直结肠息肉诊断中的应用价值。方法前瞻性研究。抽取2020年7月至2023年7月商丘市第一人民医院收治的62例疑似结直肠息肉的患儿作为研究对象,病情明确之前所有患儿均开展窄带成像加放大内镜、白光内镜检查诊断,并以病理结果为标准,比较窄带成像加放大内镜与白光内镜的诊断效能。结果62例疑似患儿通过病理检查,明确为结直肠息肉者58例;对58例患儿的息肉检出情况进行分析,发现<10 mm、10~20 mm、>20 mm者分别为15、30、13例;分析58例患儿息肉的部位,发现结肠脾曲、直肠、降结肠、乙状结肠者分别为7、22、21、8例。62例疑似患儿通过窄带成像加放大内镜诊断,检出结直肠息肉者59例,确诊58例;白光内镜检出50例,确诊46例;窄带成像加放大内镜诊断准确度、灵敏度、特异度高于白光内镜(P<0.05);窄带成像加放大内镜不同大小肠息肉、不同部位肠息肉检出率高于白光内镜(P<0.05)。结论儿童直结肠息肉的诊断中,窄带成像加放大内镜的诊断效果优于白光内镜诊断,可以将其作为临床常规诊断方式,为患儿提供有效的诊治保证。Objective To compare the application value of narrow-band imaging with magnifying endoscopy and white-light endoscopy in the diagnosis of rectal polyps in children.Methods A total of 62 children with suspected colorectal polyps were prospectively selected from First People’s Hospital of Shangqiu from July 2020 to July 2023 as research objects.Before the confirmed diagnosis of the disease,all the children underwent narrow-band imaging with magnifying endoscopy and white-light endoscopy examinations,and narrow-band imaging with magnifying endoscopy and white-light endoscopy were compared in term of the diagnostic efficiency according to the pathological results.Results Among the 62 suspected children,58 cases were confirmed as colorectal polyps by pathological examination.Polyps diameter was<10 mm in 15 cases,10-20 mm in 30 cases,and>20mm in 13 cases.According to analysis of positions of polyps in the 58 children,there were 7 cases of splenic flexure,22 cases of rectum,21 cases of descending colon and 8 cases of sigmoid colon.Among the 62 suspected children,narrow-band imaging with magnifying endoscopy detected out 59 cases of colorectal polyps,including 58 cases of confirmed diagnosis.White-light endoscopy detected out 50 cases,including 46 cases of confirmed diagnosis.The diagnostic coincidence rate,sensitivity and specificity of narrow-band imaging with magnifying endoscopy were higher than those of white-light endoscopy(P<0.05).Compared with white-light endoscopy,the narrow-band imaging with magnifying endoscopy had higher detection rates for polyps with different diameters and in different locations(P<0.05).Conclusions In the diagnosis of rectal polyps in children,the diagnostic effect of narrow-band imaging with magnifying endoscopy has better diagnostic effect than white-light endoscopy;it can be used as a routine diagnostic method in clinical practice,providing effective diagnosis and treatment guarantee for children.
