司美格鲁肽联合吡格列酮二甲双胍治疗肥胖2型糖尿病的效果观察

摘　　要：目的分析司美格鲁肽联合吡格列酮二甲双胍对肥胖2型糖尿病(T2DM)的治疗效果。方法队列研究。抽取2023年1月至2024年2月郑州大学第一附属医院收治的肥胖T2DM患者68例,采用摸红蓝球法分为联合用药组(34例,采用司美格鲁肽联合吡格列酮二甲双胍治疗)与单一用药组(34例,采用吡格列酮二甲双胍治疗)。对比两组临床疗效、空腹血糖(FBG)、餐后2 h血糖(2hBG)、糖化血红蛋白(HbA1c)、血糖波动情况、胰岛β细胞功能指数(HOMA-β)、胰岛素抵抗指数(HOMA-IR)及体质指数。结果联合用药组总有效率(94.12%,26/34)高于单一用药组(76.47%,32/34),P<0.05。治疗后,联合用药组FBG、2hBG、HbA1c水平低于单一用药组(P<0.05)。治疗后,联合用药组日间血糖平均绝对差、平均血糖波动幅度低于单一用药组(P<0.05)。治疗后,联合用药组HOMA-β高于单一用药组,HOMA-IR低于单一用药组(P<0.05)。治疗后,联合用药组体质指数小于单一用药组(P<0.05)。结论司美格鲁肽联合吡格列酮二甲双胍治疗肥胖T2DM患者效果较好,可改善胰岛功能,能稳定降低血糖水平,并可改善肥胖情况。Objective To analyze the therapeutic effect of semaglutide combined with pioglitazone metaformin on obese type 2 diabetes mellitus(T2DM).Methods Sixty-eight obese T2DM patients admitted to the First Affiliated Hospital of Zhengzhou University from January 2023 to February 2024 were selected for the cohort study.And they were divided into the combination treatment group(34 cases,treated with semaglutide with pioglitazone metaformin)and the single treatment group(34 cases,treated with pioglitazone metaformin)by touching red-blue ball method.The curative effect,fasting blood glucose(FBG),2-hour postprandial blood glucose(2hBG),glycosylated hemoglobin(HbA1c),blood sugar fluctuation,homeostasis model assessment-β(HOMA-β),homeostasis model assessment-IR(HOMA-IR)and body mass index of the two groups were compared.Results The total effective rate of the combination treatment group(94.12%,26/34)was higher than that of the single treatment group(76.47%,32/34),P<0.05.After treatment,FBG,2hBG and HbA1c in the combination treatment group were lower than those in the single treatment group(P<0.05).After treatment,the average absolute difference and the average fluctuation range of blood glucose in the combination treatment group were lower than those in the single treatment group(P<0.05).After treatment,the HOMA-βin the combination treatment group was higher than that in the single treatment group,and the HOMA-IR was lower than that in the single treatment group(P<0.05).After treatment,the body mass index of the combination treatment group was lower than that of the single treatment group(P<0.05).Conclusions Semaglutide combined with pioglitazone metaformin is effective in the treatment of obese T2DM,which can improve the function of pancreatic islets,stably reduce blood sugar level and alleviate obesity.
