麻黄连翘赤小豆汤加减联合血液透析滤过治疗慢性肾功能衰竭合并皮肤瘙痒的效果观察

摘　　要：目的观察麻黄连翘赤小豆汤加减联合血液透析滤过治疗慢性肾功能衰竭(CRF)合并皮肤瘙痒的效果。方法队列研究。抽取2020年3月至2022年10月漯河市中医院收治的86例CRF合并皮肤瘙痒患者,根据随机数字表法将患者分为单一组(43例)和联合组(43例)。单一组进行血液透析滤过治疗,联合组在血液透析滤过基础上加用麻黄连翘赤小豆汤加减治疗。比较两组治疗前后皮肤瘙痒评分,血钙、血镁、血磷及甲状旁腺素水平,观察并记录两组临床疗效和不良反应。结果联合组临床有效率(95.35%,41/43)高于单一组(81.40%,35/43),P<0.05。治疗后,两组皮肤瘙痒评分均降低,且联合组评分低于单一组(P<0.05);治疗后,两组血钙水平均升高,血镁、血磷及甲状旁腺素水平均下降,且联合组血钙水平高于单一组,血镁、血磷及甲状旁腺素水平低于单一组(P<0.05)。两组不良反应发生率比较差异未见统计学意义(P>0.05)。结论麻黄连翘赤小豆汤加减联合血液透析滤过治疗对CRF合并皮肤瘙痒患者效果显著,可纠正体内钙磷镁离子水平紊乱,能有效改善患者皮肤瘙痒状况,且安全性较高。Objective To observe the curative effect of ephedra forsythias-rice bean decoction combined with hemodialysis filtration on chronic renal failure(CRF)complicated by pruritus.Methods Eighty-six patients with CRF complicated by pruritus treated in Luohe Hospital of Traditional Chinese Medicine from March 2020 to October 2022 were selected for the cohort study.According to the random number table method,they were divided into a single group(43 cases)and a combination group(43 cases).The single group was treated by hemodialysis filtration,and the combination group was treated with ephedra forsythias-rice bean decoction on the basis of hemodialysis filtration.The skin itch score,preoperative and postoperative levels of blood calcium,magnesium,phosphorus and parathyroid hormone were compared between the two groups.The clinical efficacy and adverse reactions of the two groups were observed and recorded.Results The clinical effective rate of the combination group(95.35%,41/43)was higher than that of the single group(81.40%,35/43),P<0.05.After treatment,the skin itch scores of both groups were decreased,and the score of the combination group was lower than that of the single group(P<0.05).After treatment,the levels of serum calcium of both groups increased,and the levels of serum magnesium,phosphorus and parathyroid hormone were decreased(P<0.05).The serum calcium level of the combination group was higher than that of the single group,while the levels of serum magnesium,phosphorus and parathyroid hormone were lower than those of the single group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Ephedra forsythias-rice bean decoction combined with hemodialysis filtration has a significant effect in the treatment of CRF complicated by pruritus;this treatment protocol can correct the disturbance of calcium,phosphorus and magnesium ion in the body,and can effectively alleviate the pruritus of patients,with high safety.
