摘 要:目的:探讨橙皮苷在兔眼青光眼滤过术后对滤过泡瘢痕化的抑制作用。方法:选取健康成年新西兰大白兔54只(108眼),雄性,构建青光眼滤过手术模型。将大白兔随机分成橙皮苷组、丝裂霉素C组及生理盐水组,每组18只。各组分别将浸有橙皮苷(1.92 mg·mL^(-1))、丝裂霉素C(0.2 mg·mL^(-1))、生理盐水的棉片放置于筋膜囊、巩膜瓣下,均于3 min后取出棉片,生理盐水充分冲洗。术后观察眼前节及滤过泡形态,监测眼压变化。于术后7、14、28 d每组随机取6只兔处死,取出眼球,用于病理学检测,观察滤过通道瘢痕增生情况。结果:橙皮苷组和丝裂霉素C组滤过术后7、14、21、28 d眼压均显著低于生理盐水组,差异有统计学意义(P<0.05)。与生理盐水组比较,橙皮苷组和丝裂霉素C组的房水滤过更强,滤过泡更加弥散隆起;随着时间延长,各组滤过泡均较前逐渐缩小并更加趋于扁平,生理盐水组的这种趋势更明显。滤过口病理检查结果显示,术后7、14、28 d橙皮苷组和丝裂霉素C组均可见开放性滤过通道,存在少许炎症细胞和成纤维细胞,而生理盐水组滤过通道逐渐关闭,见大量炎症细胞浸润及成纤维细胞增生。结论:橙皮苷可能作为一种有效的抗瘢痕药物,可以减少兔青光眼滤过手术切口瘢痕形成,提高手术成功率。Objective:To investigate the inhibitory effect of hesperidin on follicular scarring after glaucoma filtration surgery in rabbit eyes.Methods:Fifty-four healthy adult New Zealand white rabbits with 108 eyes(male)were selected to construct a glaucoma filtration surgery model.Rabbits were randomly divided into hesperidin group,mitomycin C group and normal saline group,with 18 rabbits in each group.In each group,1.92 mg·mL^(-1)hesperidin cotton tablets,0.2 mg·mL^(-1)mitomycin C cotton tablets and normal saline cotton tablets were placed under fascial sac and scleral flap,respectively.The cotton tablets were removed 3 minutes later and fully washed with normal saline.The morphology of anterior segment and filtering bleb were observed and the changes of IOP were monitored.At 7,14 and 28 days after operation,6 rabbits were killed randomly in each group,and 12 eyeballs were taken for pathological examination to observe the scar hyperplasia of the filtration channels.Results:Intraocular pressure significantly decreased in the hesperidin group and mitomycin C group compared with the saline group at all time points after filtration surgery,the differences were statistically significant(P<0.05).Compared with the control group,the hesperidin group and mitomycin C group exhibited stronger filtration and more diffuse and elevated filtering blebs.Over time,the filtering bleb gradually decreased and became flatter in all groups.This trend was more obvious in the saline group.Pathological sections of the filtration port showed consistently open filtration channels in the hesperidin group and mitomycin C group at 7,14,and 28 days after surgery,and a few inflammatory cells and fibroblasts were observed.However,the filtration channels in the saline group gradually closed,and substantial inflammatory cell infiltration and fibroblast hyperplasia were observed.Conclusion:Hesperidin may be an effective anti-scarring drug that provides good surgical results in experimental rabbit glaucoma filtration surgery.
