超声心动图二维斑点追踪显像对阵发性房颤患者首次经导管射频消融术后复发预测价值

摘　　要：目的:探究二维斑点追踪显像对阵发性心房颤动患者首次射频消融术后心律失常复发的临床意义。方法:选取2022年7月~2023年8月在本院就诊的70例阵发性房颤患者为研究对象,患者均接受经导管射频消融术治疗,并依据治疗后复发情况分为房颤未复发和复发组,两组各35例;研究选择同期健康人群70例为对照组。对比三组的左心房结构及超声心动图参数以及作ROC曲线图分析术后复发的预测价值。结果:术后,房颤未复发组的LAAPD、LAUDD、LALRD、LAV_(min)、LAV_(max)、LAVp较房颤复发组的值低,而LAEF/%、LAAEF/%、LAPEF/%均较房颤复发组高(P<0.05);根据Logistic回归分析结果建立预测模型,计算出的左心房结构、超声心动图参数联合检测预测概率ROC曲线下面积为0.882、0.833、0.757;左心房结构、超声心动图参数联合检测预测的预测概率ROC渐近95%置信区间为0.794~0.971、0.733~0.934、0.645~0.871。结论:在阵发性心房颤动患者的首次射频消融术中使用二维斑点追踪显像技术,可有效改善左心房的结构和功能,从而为疾病复发提供较高的预测价值。Objective:Exploring the clinical significance of two-dimensional spot tracking imaging for the recurrence of arrhythmia in patients with paroxysmal atrial fibrillation after the first radiofrequency ablation surgery.Methods:A total of 70 patients with paroxysmal atrial fibrillation who were admitted to the hospital from July 2022 to August 2023 were selected as the study subjects.All patients underwent transcatheter radiofrequency ablation treatment and were divided into two groups based on the recurrence of atrial fibrillation after treatment,with 35 patients in each group;The study selected 70 healthy individuals from the same period as the control group.Compared the left atrial structure and echocardiography parameters of three groups,and analyzed the predictive value of postoperative recurrence using ROC curve analysis.Results:After surgery,the values of LAAPD,LAUDD,LALRD,LAV_(min),LAV_(max),and LAVp in the non recurrent atrial fibrillation group were lower than those in the recurrent atrial fibrillation group,while LAEF/%,LAAEF/%,and LAPEF/%were higher than those in the recurrent atrial fibrillation group(P<0.05);Based on the results of Logistic regression analysis,a prediction model was established,and the calculated combined detection probabilities of left atrial structure and echocardiogram parameters were 0.882,0.833,and 0.757 under the ROC curve;The ROC asymptotic 95%confidence intervals for predicting the combined detection of left atrial structure and echocardiographic parameters were 0.794-0.971,0.733-0.934,and 0.645-0.871.Conclusion:The use of two-dimensional spot tracking imaging in the first radiofrequency ablation of paroxysmal atrial fibrillation patients can effectively improve the structure and function of the left atrium,thereby providing high predictive value for disease recurrence.
