摘　　要：目的探讨胃原发性淋巴瘤的多层螺旋CT表现。方法回顾分析本院2012年1月-2015年12月经手术病理证实的21例胃原发性淋巴瘤的临床及影像学资料,分析病变的部位、范围、厚度、强化程度、胃周脂肪层、胃周淋巴结肿大及远处转移情况。结果21例中2例(9.5%)发生于胃底部,胃体部8例(38.1%),胃窦部5例(23.8%),累及全胃6例(28.6%)。胃壁弥漫性增厚12例(57.1%),节段性胃壁增厚7例(33.3%),局限性胃壁增厚2例(9.5%)。CT上表现为胃壁的节段性或广泛性增厚,胃壁增厚平均达21.5(s=9.4)mm;增强后示轻度强化。胃周脂肪层清晰。胃周淋巴结的肿大,15例(71.4%)有一处或一处以上淋巴结肿大,3例(14.3%)有肾门水平以下腹膜后淋巴结的肿大,2例(9.5%)合并脾肿大;1例合并脾脏受侵,1例合并肝脏浸润。结论胃原发性淋巴瘤的CT表现有一定特征性,CT扫描能够为本病的诊断提供重要的信息。Objective To investigate CT findings of primary gastric lymphoma.Methods CT findings in 21 patients with pathologically proved primary gastric lymphoma were retrospectively analyzed from January 2012 to December 2015.Observe the site of lesion,extent,thickness of gastric wall,degree of enhancement,perigastric fat layer,the encroachment of perigastric lymph nodes and distant metastases.Results In 21 cases of primary gastric lymphoma,the bottom of stomach in 2 cases(9.5%),gastric body in 8 cases(38.1%),gastric antrum in 5 cases(23.8%),the whole stomach in six cases(28.6%)were found.CT imaging showed gastric wall thickened diffusely in 12 cases(57.1%),segmental gastric wall thickened in 7 cases(33.3%)and locally in 2 cases(9.5%).CT imaging showed gastric wall thickened segmentally or generalizedly with an average of 21.5(s=9.4)mm.Homogeneous enhancements of the lesions were found in all 21 cases after contrast medium injection..Perigastric fat layer of all cases was clear.15 cases(71.4%)had one or more than one lymph nodes,among them 3 cases(14.3%)had retroperitoneal lymph nodes below renal hilum.2 cases(9.5%)combined splenomegaly and 1 patient with spleen involvement,1 patient with liver infiltration.Conclusion CT findings of primary gastric lymphoma are certainly specific,and CT scans can provide important information for the diagnosis of this disease.
