肾康注射液联合雷公藤多苷片治疗小鼠慢性肾炎的疗效及作用机制

摘  要:目的探讨肾康注射液联合雷公藤多苷片治疗小鼠慢性肾炎的疗效及其相关机制。方法将50只ICR小鼠随机分为对照组(生理盐水处理)、模型组[阿霉素(DOX)处理构建慢性肾炎模型]、雷公藤多苷片组(构建慢性肾炎模型+雷公藤多苷片溶液灌胃)、肾康注射液组(构建慢性肾炎模型+尾静脉注射肾康注射液)和联合治疗组(构建慢性肾炎模型+雷公藤多苷片灌胃+尾静脉注射肾康注射液),每组10只。检测5组小鼠的肾功能指标[血清肌酐(Scr)、尿素氮(BUN)、白蛋白(ALB)]、炎症因子水平[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、氧化应激指标[谷胱甘肽(GSH)、超氧化物歧化酶(SOD)、丙二醛(MDA)、髓过氧化物酶(MPO)]和肾脏病理变化,免疫组织化学法分析肾脏组织纤维化程度,蛋白质印迹法进行磷酸化磷脂酰肌醇3-激酶(p-PI3K)、磷脂酰肌醇3-激酶(PI3K)、磷酸化蛋白激酶B(p-AKT)、蛋白激酶B(AKT)蛋白定量分析。结果与模型组相比,雷公藤多苷片组、肾康注射液组、联合治疗组的肾功能指标(Scr、BUN、ALB)、炎症因子水平(IL-6、TNF-α)和氧化应激状态(GSH、SOD、MDA、MPO)均有所缓解,肾脏病理变化减轻(P<0.01);且联合治疗组比雷公藤多苷片组、肾康注射液组改善效果更好(P<0.01)。与模型组相比,雷公藤多苷片组、肾康注射液组、联合治疗组肾脏组织中p-PI3K/PI3K与p-AKT/AKT水平明显降低,且联合治疗组降低更明显(P<0.01)。结论肾康注射液可增强雷公藤多苷片对小鼠慢性肾炎的治疗效果,其作用机制可能在于抑制炎症反应、降低氧化应激损伤并调节PI3K/AKT信号通路。Objective To explore the efficacy and related mechanisms of Shenkang Injection combined with Tripterygium wilfordii polyglycoside(TWP)tablets in the treatment of chronic glomerulonephritis(CGN)in mice.Methods The mouse model of CGN in this study was established via tail vein injection of doxorubicin(DOX).A total of 50 Institute of Cancer Research(ICR)mice were randomly divided into control group(treated with normal saline),model group(treated with normal saline after CGN modeling),TWP tablet group(treated with intragastric administration of TWP tablet solution after CGN modeling),Shenkang Injection group(treated with tail vein injection of Shenkang Injection after CGN modeling),and combined treatment group(treated with intragastric administration of TWP tablets plus tail vein injection of Shenkang Injection after CGN modeling),with 10 mice in each group.Renal function indexes[serum creatinine(Scr),blood urea nitrogen(BUN),albumin(ALB)],levels of inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)],oxidative stress indexes[glutathione(GSH),superoxide dismutase(SOD),malondialdehyde(MDA),myeloperoxidase(MPO)]and renal pathological changes of the five groups of mice were detected.Immunohistochemical method was used to analyze and observe the degree of renal tissue fibrosis.Western blotting was used for protein quantitative analysis of[phosphorylated phosphatidylinositol 3-kinase(p-PI3K),phosphatidylinositol 3-kinase(PI3K),phosphorylated protein kinase B(p-AKT),protein kinase B(AKT)].Results Compared with the model group,the renal function indexes(Scr,BUN,ALB),inflammation(IL-6,TNF-α)and oxidative stress status(GSH,SOD,MDA,MPO)in the TWP tablet group,Shenkang Injection group and combined treatment group were all alleviated,and renal pathological changes were reduced(P<0.01);And the improvement in the combined treatment group was more obvious than that in the TWP tablet group and Shenkang Injection group(P<0.01).Compared with the model group,the levels of p-PI3K/PI3K and p-AKT/AKT in renal tissue
