郑氏中医结合智能肩关节康复机器人治疗粘连性肩关节囊炎的临床分析

摘　　要：目的探究郑氏中医疗法结合智能肩关节康复机器人治疗粘连性肩关节囊炎(AC)的临床效果。方法选取2023年1月至2024年6月于四川省骨科医院就诊的108例AC患者为研究对象,按照随机数字表法分为对照组和观察组,每组54例。对照组予以郑氏中医疗法,观察组在对照组基础上结合智能肩关节康复机器人治疗,两组均治疗(14±2)d。比较两组患者治疗前后中医证候积分、疼痛程度[采用McGiLL疼痛量表(MPQ)]、肩关节功能[采用Constant-Murley肩关节评分(CMS)量表]、肩关节活动度及临床疗效。结果观察组总有效率(98.15%)高于对照组(87.04%)(P<0.05);两组治疗14 d后主证候与次证候积分,MPQ评分中视觉模拟评分(VAS)、现在疼痛情况(PPI)、疼痛评级指数(PRI)及总分均低于治疗前(P<0.05),且观察组上述评分均低于对照组(P<0.05);两组治疗14 d后CMS量表中疼痛程度、肌力、肩关节活动度、日常生活能力4个维度评分及总分与肩关节中前屈、外展、后伸及外旋活动度均高于治疗前(P<0.05),且观察组上述指标均高于对照组(P<0.05)。结论郑氏中医疗法结合智能肩关节康复机器人治疗AC疗效确切,可改善患者肩关节疼痛程度,提高肩关节活动度,恢复肩关节功能。Objective To explore the clinical effect of Zheng's traditional Chinese medicine(TCM)therapy combined with intelligent shoulder joint rehabilitation robot in the treatment of adhesive capsulitis(AC).Methods A total of 108 patients with AC in Sichuan Province Orthopedic Hospital from January 2023 to June 2024 were selected as the study objects.They were divided into a control group(n=54)and an observation group(n=54)according to random number table method.The control group was treated with Zheng's TCM therapy,while the observation group was treated with intelligent shoulder joint rehabilitation robot in addition to Zheng's TCM therapy.Both groups were treated for(14±2)d.The TCM syndrome score,pain degree[assessed by McGiLL Pain Questionnaire(MPQ)],shoulder joint function[assessed by Constant-Murley score(CMS)],and shoulder joint range of motion(ROM)before and after treatment,and clinical efficacy were compared between the two groups.Results The total effective rate in the observation group(98.15%)was higher than that in the control group(87.04%)(P<0.05).After 14 d of treatment,the scores of primary and secondary TCM syndromes,and the scores of Visual Analogue Scale(VAS),present pain intensity,(PPI)and pain rating index(PRI)in MPQ,and the total score of MPQ were decreased in both groups,and the above scores in the observation group were lower than those in the control group(P<0.05).After 14 d of treatment,the scores of each item in CMS(pain,muscle strength,joint ROM,and activity of daily living),the total score of CMS,and the ROM of flexion,abduction,extension and external rotation were increased in both groups,and the above indicators in the observation group were higher than those in the control group(P<0.05).Conclusion Zheng's TCM therapy combined with intelligent shoulder joint rehabilitation robot is effective in the treatment of AC,which can effectively relieve the degree of shoulder pain,enhance the range of motion,and then restore the shoulder function.
