IGFBP-3、RANTES和Eotaxin-3与腰痛患者功能障碍及疼痛评分的关系研究

摘　　要：目的探讨血清胰岛素样生长因子结合蛋白3(IGFBP-3)、活化T细胞趋化因子(RANTES)、中嗜酸性粒细胞趋化因子3(Eotaxin-3)水平与腰痛患者功能障碍及疼痛评分的相关性。方法选取2022年3月至2023年3月黑龙江中医药大学附属第一医院接诊的128例腰痛患者作为试验组,另选取105例同期在本院体检的健康者作为对照组。采用ELISA法检测血清IGFBP-3、RANTES、Eotaxin-3表达水平;采用Pearson法分析腰痛患者血清IGFBP-3、RANTES、Eotaxin-3水平与功能障碍及疼痛评分的相关性;多因素Logistic回归分析影响腰痛发生的相关因素。结果试验组中酗酒史、抑郁、焦虑、体力活动高强度占比及血清RANTES、Eotaxin-3水平高于对照组,而血清IGFBP-3水平低于对照组(P<0.05);试验组Oswestry腰痛功能障碍指数(ODI)、视觉模拟疼痛评分(VAS)均高于对照组(P<0.05);患者血清RANTES、Eotaxin-3水平与ODI、VAS呈正相关,IGFBP-3水平与ODI、VAS呈负相关(P<0.05);IGFBP-3、RANTES、Eotaxin-3、酗酒史、抑郁、体力活动强度为腰痛发生的影响因素(P<0.05)。结论腰痛患者血清IGFBP-3水平明显降低,RANTES、Eotaxin-3水平明显升高,且与患者功能障碍及疼痛评分密切相关。Objective To explore the correlation between serum levels of insulin-like growth factor binding protein-3(IGFBP-3),regulated upon activation normal T cell expressed and secreted(RANTES),and mesosinophil chemokine 3(Eotaxin-3)and dysfunction and pain score in patients with low back pain.Methods A total of 128 patients with lower back pain who were treated in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from March 2022 to March 2023 were selected as the experimental group,and 105 healthy subjects who underwent physical examination in the hospital during the same period were selected as the control group.Enzyme-linked immunosorbent assay(ELISA)was applied to detect the expression levels of serum IGFBP-3,RANTES,and Eotaxin-3.Pearson method was applied to analyze the correlation between serum levels of IGFBP-3,RANTES,and Eotaxin-3 and functional impairment and pain score in patients with low back pain.Multivariate Logistic regression was applied to analyze the relevant factors affecting the occurrence of low back pain.Results The proportions of history of alcohol abuse,depression,anxiety,and high-intensity physical activity,and serum RANTES and Eotaxin-3 levels in the experimental group were higher than those in the control group,while the serum IGFBP-3 level in the experimental group was lower than that in the control group(P<0.05).The Oswestry Disability Index(ODI)and Visual Analogue Scale(VAS)score in the experimental group were higher than those in the control group(P<0.05).The serum levels of RANTES and Eotaxin-3 in patients with low back pain were positively correlated with ODI and VAS score,while the serum level of IGFBP-3 was negatively correlated with ODI and VAS score(P<0.05).The serum IGFBP-3,RANTES,and Eotaxin-3,history of alcohol abuse,depression,and physical activity intensity were the influencing factors for the occurrence of lower back pain(P<0.05).Conclusion The serum IGFBP-3 level is greatly reduced,while the serum RANTES and Eotaxin-3 levels are greatly increased in p
