基于MCU探讨防风汤加减联合推拿治疗风寒湿痹型颈型颈椎病的临床研究

摘　　要：目的:基于颈椎多功能训练评估系统(MCU)探讨防风汤加减联合推拿治疗风寒湿痹型颈型颈椎病的临床疗效。方法:选取2022年1月~2023年6月期间就诊于福建中医药大学附属康复医院的风寒湿痹型颈型颈椎病患者共70例,根据随机数字表分为治疗组及对照组各35例。治疗组予口服防风汤加减联合推拿治疗,对照组予常规推拿治疗。比较观察两组患者治疗前后疼痛视觉模拟评分法(VAS评分)、颈椎功能障碍指数(NDI)量表、临床疗效及颈椎各方向活动度和肌力情况。结果:治疗后,两组患者VAS评分、NDI评分较治疗前增加,治疗组VAS评分和NDI评分高于对照组(P<0.05);治疗后,两组患者颈椎各方向活动度和颈肌最大肌力较治疗前均有不同程度增加(P<0.05),治疗组颈椎各方向活动度及颈椎前屈、后伸、右侧屈方向的最大肌力均优于对照组(P<0.05);治疗组总有效率高于对照组(P<0.05)。结论:防风汤加减联合推拿可显著改善风寒湿痹型颈型颈椎病患者的疼痛、颈椎活动功能,增强颈椎周围肌群的肌力,从而增强颈椎的稳定性,缓解临床症状,其疗效优于单纯推拿治疗。Objective:To explore the clinical efficacy of Fangfeng Tang plus reduction combined with massage in the treatment of wind-cold-damp paralytic cervical cervical spondylosis based on the cervical spine multifunctional training and evaluation system(MCU).Methods:A total of 70 patients with wind-cold-dampness paralytic cervical cervical spondylosis who visited the Rehabilita-tion Hospital of Fujian University of Traditional Chinese Medicine from January 2022 to June 2023 were selected,and were divided into 35 cases each in the treatment group and the control group according to the randomized numerical table.The treatment group was given Fangfeng Tang plus reduction combined with massage therapy,and the control group was given conventional massage therapy.Before and after treatment,the two groups were compared and observed the pain visual analog scale(VAS),cervical spine dysfunction index(NDI)scale,clinical efficacy and cervical spine mobility and muscle strength in all directions.Results:After treat-ment,the VAS score and NDI score of both groups of patients increased compared to before treatment,and the VAS score and NDI score of the treatment group were higher than those of the control group(P<0.05);After treatment,both groups of patients showed varying degrees of increase in cervical spine range of motion and maximum cervical muscle strength compared to before treatment(P<0.05).The treatment group had better cervical spine range of motion in all directions and maximum muscle strength in flexion,extension,and right flexion directions than the control group(P<0.05);The total effective rate of the treatment group was higher than that of the control group(P<0.05).Conclusion:Combining the addition and subtraction of Fangfeng Tang with acupressure can significantly improve the pain and cervical spine function of patients with wind-cold-damp paralysis type cervical cervical spondylo-sis,and enhance the muscle strength of the peripheral muscles of cervical spine.It can improve the pain and cervical spine function,enhance
