针刺脾经组穴对中风后吞咽障碍康复治疗促进作用观察

摘　　要：目的:观察针刺脾经组穴(太白、商丘)对中风后吞咽障(Dysphagia after stroke,DAS)康复治疗疗效的促进作用。方法:将60例DAS患者随机分为试验组、对照组,各30例,两组均予以常规针刺治疗及吞咽康复训练,试验组结合针刺脾经组穴(太白、商丘)。两组均每周针刺治疗6次,4周为1个疗程,1个疗程结束后评定疗效。比较治疗前后两组患者洼田饮水试验评分、血红蛋白、白蛋白、中医症候积分、临床疗效。结果:治疗后,两组患者洼田饮水试验评分均较治疗前降低(P<0.001),且试验组组优于对照组(P=0.001<0.01);两组患者血红蛋白值均较治疗前上升,试验组优于对照组(P=0.01<0.05);两组白蛋白值均较治疗前上升,试验组优于对照组(P=0.01<0.05);两组中医症候积分均较治疗前改善,试验组优于对照组(倦怠乏力:Z=1.162,P=0.032<0.05;面色萎黄:Z=1.212,P=0.007<0.05);治疗后试验组临床治愈率明显高于对照组,差异有统计学意义。结论:针刺脾经组穴能有效提升DAS的吞咽能力,提高患者的营养水平。Objective:Observing the Clinical Efficacy of Acupuncture at Points on the Spleen Meridians for Treating Dysphagia after Stroke(DAS).Methods:60 DAS patients were randomly divided into the trial group and the control group,30 cases each.Both groups were treated with conventional acupuncture and swallowing rehabilitation,the experimental group combined with acupuncture of a group of acupuncture points in the spleen meridian(Taibai,Shangqiu).Both groups were treated with acupuncture 6 times per week,4 weeks for 1 course of treatment,efficacy was assessed at the end of 1 course of treatment.The drinking water test in the depression test score,hemoglobin,albumin,TCM symptom score and clinical efficacy of the two groups before and after treatment were compared.Results:After treatment,the scores of the drinking water test in the depression of the two groups of patients were lower than before treatment(P<0.001),and the experimental group was better than the control group(P=0.001<0.01);the hemoglobin values of both groups of patients were higher than before treatment,and the experimental group was better than the control group(P=0.01<0.05);the albumin values of both groups of patients were higher than before treatment,and the experimental group was better than the control group(P=0.01<0.05);The points of both groups of traditional Chinese medicine symptoms improved compared with before treatment,and the experimental group was better than the control group(Fatigue:Z=1.162,P=0.032<0.05;Sallow complexion:Z=1.212,P=0.007<0.05);The clinical cure rate of the experimental group after treatment was significantly higher than that of the control group,and the difference was statistically significant.Conclusion:A group of acupuncture points for the spleen meridians can effectively improve the swallowing ability of DAS and improve the nutritional level of patients.
